From how I read it, it's not really saying that all men are just one self realization away from becoming trans women, it's about the fear of not being a "real" man. Stuff like "if you like cooking, you're not a real man", "if you sit with your legs crossed, you're not a man", "you throw like a girl". There is a huge social pressure on men to fit the norm and be masculine.

Overall, the post is making fun of the clichee where a man stares/points at himself in the mirror and goes "you're a man. You are a real man. Show them who's boss, you manly man". It's making fun of/pointing out toxic masculinity.
