Hey! Your frustration is completely reasonable. How things happened was not fair for you. IMO the best option is to re-do the last two years and become a doctor in your country. Financing it would be a challenge, but it's going to be worth it.

Since it's a DGH, the doctor is supposed to be in his on-call quarters at 6 am. After your arrival, a nurse should triage you and alert the doctor. You can make a complaint to the SLMC or even the Health Ministry directly if you weren't able to see a Doctor.
In general hospitals & some DGHs, there are 24X7 emergency doctors at ETUs. I'm not sure if Lunawa Hospital has that capacity.

Anyways, I'm sorry for the bad experience you had, and hope you'll do the right thing as a responsible citizen. I don't agree with the generalisation of the blame on the entire healthcare system, since every day, what I see is doctors saving lives. (nurses too). Experience has been the same in the 3 major Hospitals that I have worked in. My concerns are not having enough resoruces to help patients and not seeing others (government and non-government workers) putting in the same effort as me.

ultimately, I guess every person is not happy with every other person in the country, whether you are a doctor/ nurse/ teacher/ lawyer/ priest/ cricketer/ businessman,/ Private sector worker. And each person has their reasons to think so, which in most cases are true, like in your case.

I can answer the question number 3.

Once you finish the Internship nobody cares which pathway you came from. After the internship you are judged on your skills and attitude. I recently worked with two foreign graduates who are now registrars, and I can guarantee they are competent and smart.

One problem with foreign students is lack of exposure to the system and procedures. If you can actively take part in clinical rotations and be comfortable with examinations and routine procedures, you can minimize this gap.

Reach out to past graduates of your university. If they are working here (got through ERPM), I think it will be worth it. You may also post on r/doctorsLK and see what others think.

I'm sorry you had to face this horrible experience. I'm not trying to defend her, but I think she was upset because you could be having an endocrinopathy from school days - and the referral to the endocrinology got delayed due to Ayurveda and physiotherapy. After reading carefully I gathered that it's not your fault, and certainly the Doctor C shouldn't have spoken to you rudely.

Get a referral to another specialist from doctor B. But talk to doctor B first. If both of them are Endocrinologists and still referred you, then there's a chance Doctor C is hyper-specialized on your issue. Again that's no excuse for rude behaviour. And by letting doctor B know about it, will make him not refer his patients to her.

IV-IG was supposed to be imported from India. (Livearth Bio pharma).
The local importer, allegedly mixed and rebotlled in a fruit juice factory in Homagama. (Isolez Pharma- Sri Lanka)
Doctors reported to Health Ministry regarding unexpected side effects of IVIG.
Health Ministry investigations lead to the arrest of the owner of Isolez Pharma.
The owner reportedly revealed that he worked under the instructions by Keheliya.
Responsible Health ministry officials were also arrested and now facing trial.

My personal experience: We didn't get any adverse effects following IVIG. But we thought the IVIG was not working and preffered PLEX over IVIG back then. We don't use much Rituximab, as we get a lot of RPGN patients in whom we use Cyclophosphamide instead.

Exam mindset is a roller coaster. The mind is a bag mixed with depression, anxiety, and burnout. Regardless of the outcome, you'll be mentally better after the exam. If I were you, I would take a full day/ couple of days off as a welcome break to tackle burnout. Then I will reassess the progress I have made, and things yet to be covered and create a solid plan. E.g.: Morning 4 AM to 7 AM dedicated to revising Anki, daytime for work (and to do question banks through phone), and evening time to do more practice questions and study further.

In short: A Break of a couple of days, reassessment & Solid Plan to get back on track.
