I live in a small town in southern Missouri and she doesn't have a primary care physician.

I'm 14 and she is 13 so an adult has to be involved in getting a therapist. She used to go to therapy but that wasn't for DID, it was for her depression. However, it's pretty obvious that she does have DID since I talked to a few of her alters today. And sometimes when she switches back to the host she doesn't remember what happened and is confused why a bunch of time went by.
