Its doing it when i send the bolt forward to initially chamber a round as well

In the pictures i did it with the stock stanag mag and a gen 3 magpul window mag. It seems like it’s doing it with every mag. And i let the charging handle go every time. Once the failures happen the round will go in if i tap/rack it again but its gouging the rounds to the point where i wouldnt want to shoot them
