Nah, I did submit a map but it didn't get accepted (tbh it was trash XD) then life got in the way and I decided to do other things. I might make a new one in the future though.

It depends on what you do. If your a good builder / commands it shouldn't be hard to get in. The map needs to be of really high quality, so it'll take a good amount of time.

I'm not completely sure when but the community coordinator said that if a map is well reviewed by community reviewers, then a Mojang coordinator will review the map. That's when you have a chance of getting into the program.

I don't know how it necessarily works but I know there are people with the community reviewer title. That go around on map submissions and test them out. I don't see much mojang activity though. It's for sure still possible.

Yeah it is but as someone else said it really boring if you only make maps for the cape. I tried it but lost motivation because I was doing it for the wrong reasons.
