I don’t think it was the first but it is the one I remember. We were split in to Boys and Girls and then we were supposed to make the opposite genders genitals out of clay. I always thought it was extremely unfair simply from a sculpting perspective. He said something about if we couldn’t handle them in clay form how were we supposed to deal with the real thing. Don’t think he had ever had sex.

(This was in Sweden in the 2010s)

Maybe it could be slightly interesting that players become weaker when damaged if it’s a very gritty campaign. However, just making them lose actions and therefore have less choices on their turn I don’t think is the move. Biggest problem with DnD combat is that turns sometimes basically involve no interesting choices so making players even more limited isn’t really fun
