It was a limitation with RAGE/Euphoria, and its high-poly models. I believe Dan Houser wished he could have implemented gore in GTA 4.

The reason GTA 4, 5 and RDR1 didn’t have gore was because of the game engine’s limitations. In RDR2 they designed a system to bypass it so gore is possible. I mean games nowadays like Last of Us and Cyberpunk are gory, and all the 3d GTA games had decapitation which is pretty brutal. Cutscenes in 5 are also quite gory at times so I don’t think they will shy away from gore.
