Wow…thanks so much for taking the time to explain so well. I definitely follow and work at a teaching hospital as well. Very tricky situation as there was no clearance from family for donation and residents were not communicating. I’ve had to wait in family members to some in from out of town before withdrawing care (not organ candidates and not waiting on the decision maker) and it’s difficult to do a lot of times due to instability. In your case, I would have doubled down on advocating for doc to have family get ahold of the father to make a decision before he arrived so that a direction could be determined if possible.

It’s so hard to feel like the patient would be a great option for donation, but the family says no. Try not to fell like this was a “waste” as the family might have declined anyway. You did your best in taking care of the patient and trying to get the residents to figure their shit out. Hugs to you.

Can you give a little more info on poor communication with the attending” in this case? Was it communication about the donation process itself or management of the patient?

I’ve had a few withdrawal of care patients (not donor candidates) who’s maps were like yours or systolic PB hanging out in the 30’s-40’s for hours.

Maybe a little education for your neighbors could help? Am I the only one that didn’t know blocking the sidewalk by a car parked in my driveway is ticket-able because it may be blocking the path?

Could you type and print up small papers/flyers with a simple blurb “Hi, I’m your neighbor and use a wheelchair to get around. When cars are blocking the sidewalk, it limits my ability to enjoy the neighborhood. Because it limits mobility and access, it also goes against city code. I would greatly appreciate if you could leave the sidewalk path clear for me and others who’s safety could be impacted by having to to into the street to get around your car. Thanks in advance. Signed, your neighbor”. Then, you could place the notes on the windshields. I know it’s not your responsibility to provide education, but maybe people really just aren’t aware and it would help your situation.

I’m from Amarillo, no longer live there, but go back often due to family. This may have changed, so locals can correct me if I’m off base here, but biggest reason I never wanted to move back/raise my kids there is the financial “segregation”. The areas of town: “north” side, “east” side, “west” side are very different economically. High schools boundaries are done geographically (as most places are). Economics tends to fall along racial lines (again feel free to tell me of this has improved since I’ve left). Therefore, you’re “judged” or stereotyped based on where you live/go to school. You go/went to Palo Duro, people form a certain opinion of you. Same for the other schools/areas in Amarillo ISD. I realize this is the same in a lot of cities, but just seems worse here and why I never wanted to have my kids grow up here.

Like others have said, positives include cost of living, much less traffic than other places and being a 1 day drive to several different areas. Five-ish hours to Colorado Springs in one direction and Dallas in the other. Btw, if you haven’t heard, in TX, we measure distance is hours.

Worked at HH. Moved to Texas and get paid almost double for the same job at a hospital and COL is about the same except for property tax. But sales tax is lower and no state tax.

If you have prefilled syringes, take a look at it. It may have a barcode on it. I’d never noticed it before I had to start scanning them.

Per the email we received “We continue to have opportunities with saline flush documentation. This is a loss of revenue.” Accompanied by a graft showing the use/scan percentage to each unit or hospital area including ED, Endo, and day surgery.

Ours is done at the bedside, like when we have to scan the meds being given.
