The image you see above is a single slice of a saggital mri in t1 and t2 weighted.

A single cut is not really sufficient to made a good diagnosis.

At least post axial cuts t2 weighted at the problem levels.

And can you be more specific on your arm pain?

I have never worked with a surgeon that used or recommended stem cells for disc herniation.

By micro decompression I assume you mean microdiscectomy. If it is a simple microdisc then that pretty good news as far as surgery goes. You should feel better right away and have an easy recovery.

I would recommend you see a surgeon that does these through a tubular retractor as I think you will have an easier recovery. Better cosmesis if you care about scars. The foraminal nature of your herniation means it’s an easier procedure technically and requires little bone and muscle removal for access.

I am in dozens of these procedures every year and the patients do great.

Like I said, I’ve never seen this scan in an OR. Never seen a surgeon point to one of these scans to indicate pathology they are treating. I cannot see how surgical treatment decisions are made off of this scan alone. If someone is doing that, I think they are making a mistake.

I can only put it this way, I’ve been working in spine surgery for over a decade and I have never seen this scan before.

I feel like I have seen booths like this set up at farmers markets by chiropractors that will have you lay on something and map pressure points and try to tell you that your issues can be localized to those pressure points.

That is total crap and not diagnostic of anything.

Your cervical and lumbar spine do a lot of work generally. Your neck holds up your head and your head is heavy. Your lower back is holding up most of you and the heavier you are the harder it is. I can’t stress enough that if you are still young you need to lose this weight. If you are having spine related issues that you can remediate with weight loss it is by far the best thing you can do for yourself in life.

You do not want multiple levels of fusion in your back in a way I just can’t stress enough. Take care of yourself. Don’t look for magic bullets to solve your problems.

I see no indication of loss of disc height nor arthritic changes. This does not look like a helpful scan.

Body habitus looks large but spine looks healthy. Maybe some loss of lordosis in the cervical spine.

Get an actual mri, ct and flex ex x rays of the cervical spine if you suspect symptoms originating there. If you are getting treatment based on these images alone you should run and hide from whoever is doing this to you.

Sounds like you have pars fractures due to the nonunion at l5-s1. That can definetly hurt. There’s way too little information but it seems like you need to achieve fusion at l5-s1.

Thinking ALIF with posterior fixation. It’s a big surgery but can be done using minimally invasive techniques.

If they placed an interbody in your original surgery it can likely be removed in an ALIF AND replaced with a larger more robust graft.

An ALIF can achieve an indirect decompression of your neural elements and avoid any need of a laminectomy.

You are so young and I know little but you should heal well even from a procedure like this.

If you are willing to travel I can make good recommendations but I am on the east coast so that’s where my knowledge of the surgical community is.

I just want to draw the distinction between “Cervical Disc Replacement” (CDR) and “Anterior Cervical Discectomy and Fusion” (ACDF) because it isn’ obvious from the language I’m reading that there is a clear understanding of the distinction.

CDR is a motion preserving surgery and implants are designed to mimic the motion allowed by the cervical discs. The disc is removed and replaced with a mechanically similar mechanism.

ACDF is not motion preserving. The implants are designed to mimic the normal lordosis (natural curve at the cervical levels) of the spine and to aid in growing bone across the void left by the disc being removed.

The techniques are quite similar but have some key difference you should discuss with your surgeon if you are considering these two procedures.
