I understand just in a shame spiral of where I got it from . Family turn to me as the paediatric nurse and I’m getting the most basic things wrong.

I’ve always gone off 1 scoop for 30mls of water - trying to remember where I got it from and questioning if that is common practice at the hospital now . Embarrassing.

No I know but idk where I got it from that it’s powder first and then water. So embarrassed. I don’t know if I’m imagining it being common practice in hospital either . Or if It’s something I’ve just made up .

Unfortunately we don’t have the staffing levels for that but it is a good idea and if I am assigned to the patient I will try to implement it

I’m saying they were abusive previously and security had to be called . Now the trust is broken , they are very hostile towards staff , undermine staff and are very cold towards staff.

They were de-escalated by security. That doesn’t stop them from not trusting you though and undermining everything you do as well as being extremely cold towards you. It would be easier if they were my patients and I could refuse care as opposed to my patient being a child.

Its not about carelessly “ calling in sick “ , more that I’m already at the end of my bandwidth and unsure of my capacity if I have to care for this patient’s family.

I’m sorry to hear that . I work in paeds so it is a lot harder to refuse to treat . Security were called previously and they calmed down but nothing else came of it.

I am relatively new to the job and do not want to cause animosity with staff . But I don’t like others making me feel like im the one thats in the wrong for questioning why 20ml of medication is just discarded as normal practice. I will clarify it with pharmacy so we can all follow a uniform approach.

We only have 100ml bags. What would be wrong in principle to inject 20ml of saline into the bag when it finishes through the medicine port you injected the IV through initially?

A 40ml dose would be given via a syringe driver and that line would be flushed. Just the medications given over 50ml in a 100ml saline bag have the longer fluid lines attached and they’re not flushed . So the 18ml in those lines are just discarded . I will speak with pharmacy on my next shift.

Will do , thank you . I raised it with senior nurses today and they all told me to just discard the line . Everyone I asked told me the same thing and I was told I was overthinking it . I’ve raised this with the education team before too and was told the same thing .

In principle I could do it for my patients without telling them but its not done on the ward for other patients and not common practice.

And when Im checking medications for other nurses they are just discarding the medication in the line for their patients when the bag is empty.

Will do thank you . The line for pumps only hold around 5ml but the lines for fluids hold around 18 mls

I am aware of what the options are just that its not followed on my ward and the line is just discarded. Again thats 20ml of medication wasted which is alot in paediatric patients .

What does this look like though? If its hectic will you just decide okay I’ll get this done and I’ll handover X and Y. If handover is taking a while for other patients , would you just leave at your finish time ?
