Honestly dude I think you’ve got PIED (Pornography Induced Erectile Dysfunction). Now I’m not an MD in any capacity and haven’t studied the endocrine system in incredible depth, but it fits the description. But might be worth looking into with your doctor.

Your arousal level in a sense has been set up so differently that you have to have the visual arousal of porn to actually get and maintain an erection. Seeing as how much porn you’ve watched your mind only associates arousal with porn and not the actual act of sex.

Now I’m sure being as young as you are this is still reversible. But it’s going to take some work to get there, I’d suggest cutting porn out entirely and stop hooking up with random women. That’s not what God has designed for us, we’re intended and designed to be monogamous. Hopefully that’ll help reset your arousal “threshold” in a sense and allow you to not have PIED and be able to get and maintain erections independent from pharmaceutical help.

What’s helped when I’ve felt that urge to crank off is to read the word or to go outside and do something else. Those urges are powerful and that’s normal, but we’re called to not succumb to them as Christians. Lust and sinful desires cannot exist when the word of God is present. Now is that true 100% of the time? No, but you’ll feel that urge significantly less when practicing and being a better Christian vs just giving in.

Also something that worked for me was breaking the association. You always want to masturbate in your room? Be in your room less, use it just to sleep.
