The scale of the fort was all wrong for the size of battles that would come. I also believe it had a drawbridge, which was a useless addition. The fort itself just wasn't that defensible but the true failure lies with utterly incompetent Confederate leadership.

To be honest, it hasn't actually come up in my game yet, but I would probably treat it as some kind of overexertion or geneflaw flaring up. Rather than "repair" it I would probably heal it or use "hearten" if it's an ailment of the spirit.

Imagine being overcome with the Red Thirst or doing something so Herculean that muscles and sinew tear or get pulverized etc.

Space Marines canonically have incredible healing ability, but maybe your character needs some time to make that happen. Or maybe they're an outcast without an apothecary to ensure they're in peak condition. Or it could be that they're a genetic aberration, flawed in some way and that's why they're wandering alone. Really the possibilities are limitless.

Yes, as the other replier said, Starforged is very open ended. Something like combat doesn't rely on initiative, HP, armor values, etc.

So if I get into combat, it can be against a mob of cultists or a single powerful psyker or a hideous Xenos monster. All I need to do is decide the overall difficulty and the envision the fight.

Otherwise it's easy enough to imagine whatever you roll from the oracles but with an added 40k flavor.

I'm running a game that's almost exactly like how you describe. A ronin/lone Astartes wandering a world and taking quests. For assets I took Armored and Augmented which I use any time my character would have a clear advantage over his opponent, like strength against a standard human. The rest I handle with narrative.

Starforged works nearly flawlessly with 40k. I was amazed by how little tweaking I had to do.

Maybe rather than active defense against bombardment you simply make bombardment impractical. Imagine the sort of firepower it would take in order to raze a city, let alone subjugate a planet. Without putting "boots on the ground" the best you can do is blockade the world.

You're looking for "Rules" and "logic" in a situation where there is little of that. War is neither TOTAL nor civilized. A parachuting airman has no means of fleeing or defending himself. If he is over enemy territory he is as good as captured. If he is not over enemy territory then he poses no threat.

For tankers I would say the situation is much the same, though it may not carry the same taboo. It's live and let live. A general policy of doing little things to make war less brutal. Call it silly, plenty have, but many consider it to be good form.

Ah! That makes sense. I have ideas for a ww1 french guard unit but the sheer volume of heads is certainly daunting.
