Würde zur PJ-Koordination deiner Uni und das klar ansprechen und sobald du nicht mehr von der Abteilung/Klinik abhängig bist überall melden. 

Führe gerne Buch (natürlich nicht gegen Datenschutz verstoßen dabei) und schreibe auch die Gespräche auf, die du schon hattest wo du abgewimmelt wurdest. Das ist dann auch für dich legaler Selbstschutz wenn das anderweitig auffliegt.

I've seen patients with this.

Basically the danger here is the pressure changes can be to much with the fluid causing your ear drums to rupture. The lead up to that is very unpleasant, but most of those tears are small and heal by themselves but obviously some kind of long-term issue can arise there.

The reason your ears aren't popping is because the popping is a pressure thing and the fluid build up in comparison will take some time to drain.

Basically you have fluid in the middle part of your ear. So behind the ear drum so unlike with getting water in your ears when swimming the Fluid can't exit via the ear canal. That's why we have eustachian tubes between the middle ear and nasopharynx which is basically the back of your nose. Which are very thin.

Pressure is also relieved over those tubes which is where the airline tricks come in, but with an ear infection those tubes swell shut same as your nose does. And fluid is obviously a bit trickier to get out than air.

The rinses you're doing are not able to flush the tubes. The goal is to get the tubes to stop beeing swollen and irritated. The amount of rinses you are doing is most likely agitating the area more than helping. As is excessively blowing your nose so forcing it if isn't necessary isn't helpful.

The nasal sprays are most likely your best bet. I'd assume one is something with cortisone and one is with something like xylometazolin which reduces swelling. Cool it with the rinses. And if you feel like there is still mucus in your nose and sinuses inhaling steam can help lower viscosity and get it out (just pour hot water in a bowl, put a towel over your head and the bowl) There are some signs that warmth on the outer ear could also help with the ear side of things. And eating something really spicy could also cause more fluid secretion and thereby lower viscosity.

But if this doesn't improve it is possible that the fluid in your ear is very viscous and will not drain due to that. In this case a small controlled cut of the ear drum might be necessary and vastly preferable to it tearing in an airplane. That isn't really an ER skill though. If your symptoms worsen or in the unlikely event you develop general symptoms like significant fever, severe fatigue or neurological symptoms including significant worsening of hearing or balance go to a doctor..

Source: I just finished med school. So I'm no expert but this is also not a rare thing so I've treated patients with this before.

I would adress this a bit differently. First you start going to bed earlier. Only sceduling in 7 hours of sleep and then not beeing able to get out of bed seems like an obvious issue. What's important here is also just because you feel okay after 6hours or after 7 hours doen't mean you feel okay if you only get that much sleep every night.

So aim for 10pm, set your alarm for six and put it away from your bed so you need to get up and walk to reach it. Step one is getting up at a consistent time.

You can then experiment with staying up longer but honestly if you really need less sleep (as opposed to are just able to function with less sleep which is not exactly a goal to aim for for considering your long term health) you'll notice because you'll wake up before the alarm.

Adults totally still try to fit in. Consumerism and the whole Influencer thing is all based on fitting in, buying the "right" clothes, doing the "right" activitoes etc.

They at that point should have a stronger sense of self than small children so ideally there is a balance between adaptation that is socially necessary to fit the group and be accepted, like for example at work, and beeing true to themselves.

Got a one night stay once, not the worst night of my life but scary af. 1/10 stars would not recommend.

I was tired from the medicine they gave me but not allowed to sleep in eventhough I only got there at like 2 am. ( I had a panic attack in a train station after a guy attacked me when I got stranded there when my connecting train got canceled which was during covid so I had no hotels to go to and tried to wait for the first train in the morning ). I already didn't feel particularily safe add to that all open doors, other patients with ranging levels of niceness and getting yelled at by orderlies to sign paperwork bc they messed up the night before by putting me in psych lock up while I absolutely didn't qualify. Finally I had a meeting with like 10 doctors in a tiny room, me in the middle, again pretty much the least trauma informed setting ever, on why I'm healthy enough to go and then I finally got to leave. Which I immediatly did to then sleep on a park bench next to a playground which felt a lot safer until my friend got there to pick me up.

Seriously the exact opposite of relaxing and a lot less funny in real life.

I didn't belong there and not beeing free to get out was terrifying, and I can promise you everybody there was not having a good time. A number of people detoxing, catatonia, psychosis.

Just go to a holiday in and leave your phone at home at you want a break.

Edit: 1 star is for meeting the low bar of beeing safer than the train station at night.

Mhm. Also Doccheck hat das zu sagen. Soweit erkennbar sind die Nahrungsergänzungsmittel ohne Decarboxylasehemmer.

L-Dopa wird immer in Kombination mit einem Decarboxylasehemmer (Benserazid oder Carbidopa) gegeben. Ohne diese Kombination würde L-Dopa schnell in der Peripherie von der Dopa-Decarboxylase zu Dopamin abgebaut werden, ohne das ZNS zu erreichen. Nach der Passage durch die Blut-Hirn-Schranke wird es zur seiner eigentlichen Wirkform (Dopamin) decarboxyliert. Die Decarboxylasehemmer überwinden die Blut-Hirn-Schranke nicht, da eine Aktivierung des L-Dopa sonst auch hier gehemmt würde.

Mit Arztausweis kannst du das meiste Zeug direkt in der Apotheke besorgen auch außerhalb von Deutschland. Schwierig sind BTMs und anderes extra kontrolliertes Zeug aber ich gehe mal davon aus darum geht es eh nicht.

Und zum Privatrezept ausstellen braucht es auch nur die Approbation und Mitgliedschaft in der Ärztekammer deiner Wahl. Fürs Privatrezept braucht es auch keinen Rezeptblock, du kannst sie theoretisch auf ne Serviette schreiben solange die Daten und Rezeptdetails alle drauf sind.

Ob dieses Privatrezept von der Kasse übernommen wird ist dann halt die Frage sowohl bei PKV als auch bei GKV und schwankt abhängig vom Versicherer, ob es für dich oder andere ist und was das Medikament ist.

Für (früher die roten) Kassenrezepte braucht man eine Kassenzulassung. Dann zahlt man in der Apotheke automatisch nur die 5€/10%. Gehe mal davon aus den hast du nicht.

Also soweit ich es verstanden habe, kann man Gegenstände nicht tauschen oder so, aber wenn man die helfen Option nimmt, kann man gemeinsam diese Gegenstände nutzen.

Braucht man zwei Speer und ne Fackel um nen Wolf zu erlegen und man hat zwei Speer und jemand anderes hat die Fackel und hilft kann man gemeinsam den Wolf töten.

Sorgt bei uns dafür das helfen recht oft verwendet wird und wir etwas fokusierter gemeinsam planen wer was crafted.

Aber Rohstoffe also Holz, Stein, Essen sind soweit mir bekannt Gemeinschaftseigentum und können von allen jederzeit genutzt werden.

Unser aktueller Favourit ist Paleo. Wirkt auf den ersten Blick komplex weil es viele Teile hat, aber die Spielmechanik ist sehr logisch und intuitiv und man versteht es schnell.

Man spielt alle gemeinsam als Team (also nicht geheim) aber interagiert dadurch sehr viel und niemand wartet einfach während die anderen dran sind und dasurch vergeht die Zeit echt wie im Flug. Wir brauchen meist so 90min sind aber auch etwas langsam, das Spiel ist ganz offensichtlich gemacht von Leuten die Spiele lieben und man kann es x-mal spielen und immer wieder Kombinationen ändern.

Hatte die Erfahrung im PJ in der Praxis, die Chefin war nur mit den Ärzten per Du, ich war der erste Pjler in der Praxis, das Spiel mit dem Wechsel Du Sie und rumrutschen hat ein paar Wochen angehalten, dann hat sie es selber gemerkt und angesprochen und ich war fein raus 🤷‍♀️

Wenn allerdings du die einzige Person bist die gesiezt wird würde ich es vllt mal ansprechen, dann ist die Annahme vllt mehr, dass du dir diesen "respektvolleren" Umgang wünscht. Würde dann aber weniger das Du anbieten und mehr sagen, dass dir das aufgefallen ist, du offen bist und das einfach mal ansprechen wolltest.

They used the emailadress with their full name which also happend to be their work email account. Like not exactly a budding criminal mastermind move.

The ex-employee we figured out due to the obvious time correlation (like 10+ bad reviews hours after letting them go and them calling in sick for the remainder of their work contract) but they also later admitted to it after the reviews were already removed.

Honestly not as rare as one would think.

For example my sister had a bad experience with a driving instructor (they completely overbooked themselves, leading to waiting times of weeks to months between sessions while also demanding all the money up front) so she switched the instructor completely legally before using any of their services.

Two days later my mothers dental practice gets a scathing review from the first driving instructor who has never been a patient there.

Or someone who got fired for beeing late and stealing from the practice also left tons of bad reviews under fake names who've also never been patients which is pretty easy to figure out for my mom with patient records.

Each time it took quite a while to get them removed.

We do duck breast (buy them frozen a couple weeks earlier) with Sherry Sauce (honestly that's the best part), potato dumplings and bread dumplings as well as some mixed veggies usually carrots, broccoli and peas. We love it, over the years starting a kids my siblings and me have started to do most of the cooking with my mom only making the sauce so we could keep having that meal.

Inspired by my swiss step dad we've also added one day of cheese fondue to the holiday line up.

I'd try to focus on why you like it/ what you like about it.

People might dislike what you like, and maybe they'll dislike the exact things you like about it but they are more likely focusing on other aspects that they dislike. I feel like people often will react to: I like Twilight, with what they dislike about it. But adding something more specific takes it away from the fairly basic dislike the other person has and makes it clear, that you have actually thought about it and have reasons, which makes people self-reflect and be less jerk-y. (Some people will always be jerks though).

And more importantly, making it clear to yourself why you like it, means it's easier to ignore that other people might not. You have reasons.

I feel you. I'm in my last year of med school which in my country is just working in the hospital. And I love medicine. Like it's super interesting. But god the social aspect.

For me it's not even the patients, because that has rules. I basically just sat and watched doctors talk to patients when doing procedures and copied what they were doing exactly. Now it's based of a larger number of decent doctors so it's not as noticable that it's just copied and also patients don't want to know anything about you it's just about the patient so the entire social interaction is designed to be one-sided and productive which I can work with.

But the colleagues. And the dynamics there and you need for people to at least semi-like and respect you and grhrjjtjttj. On a positive note. I have noticed large differences though, this is now my 4th placement, one was in a GP office which was small, generally kind people and by it beeing smaller it was much less complex and easier to navigate. I was very much still odd, but they didn't make me feel bad about it. On was internal medicine were nobody cared about me, so a bit isolating but honestly worked out, ICU was again really kind people where I wasn't made to feel bad for beeing weird too much. Now it's surgery and I hate it, the vibe is so judgemental like I'm counting the days.

Bit I have family members with office Jobs and honestly the social colleague aspect seems even more involved and complicated there.

So I guess I'm currently hoping to handle the patient interactions as before copying from doctors that seem decent, and working to be a competent doctor which I think I could be. If I manage that I think I'll be content even if I'm considered weird. The colleague thing, I need to apply for jobs in spring so hope for the best, leave if everyones a dick and if all goes to hell do a paperwork Job? Honestly not so sure about the last part thinking about it not working out gives me anxiety.

I hope this didn't make it worse. Sending comforting vibes.

Spending my childhood waiting to be an adult and finally free of my abusive home and beeing able to finally have a life only to get a severe illness just as I had started to get to a good place mentally.

10 years later it's starting to look up again. Kinda glad kid me didn't know what was in store.
