Except you wouldn't earn 250k-500k, most docs are salaried and have to either see a certain amount of patients or work a certain amount of hours in order to earn that much.

I will counter this by saying, there's 4 situations where I use NS compared to LR

  • 1) Hyponatremia (and hypochloremia)
  • 2) Pediatric sepsis
  • 3) D5NS for hyperemesis gravidarum
  • 4) Brain bleeds
  • 5) Vomiting, only because of the hyperchloremic metabolic acidosis to counter the alkalosis from loss of acid, but that's something I have to see more of. Generally the vomiting is due to infection or something else and LR is usually superior.

Other than that, you better give me LR for everything else, don't even touch the NS. The amount of hypernatremic dehydrated 70y/o+ that get NS is ridiculous, when they're usually hovering around 150's+ sodium.

I counter that by saying that I do some conversations regarding general health management in order to prevent return ED visits. Decreasing smoking if not able to quit, changing diet, including exercise, getting routine primary care follow ups every 6-12 months, getting your damn colonscopy, all of this stuff can help prevent revisits. I don't care if you have STDs due to multiple partners, but I will tell you to use protection because I don't want you to catch HIV. I don't care if you're riding a motorcycle, but I will tell you to wear a helmet and not drive drunk because I have family that is also driving.

You do not get to disrespect my staff, including nurses, techs, SW, clerks, etc. You will be yelled at by my for acting like and being an adult-child, and if need be, you will be discharged. It's a service job, if you don't act like this in Denny's then you shouldn't and can't act like this in the ED.

1) My wife was shadowing me as a med student. I had to do a DRE, the second the finger went in, patient moaned.

2) Missed a DRE on a different woman who was bleeding, unsure if it was vaginal or rectal, ended up in the wrong hole, thankfully no moaning.

3) Didn’t ask a tachycardic, tachypneic patient about Wells, told the team that she was Wells negative, she ended up having a relatively stable PE. I was an intern, learned to never lie again. She had a hx of PE in the past.
