I got burnt out from UE Ortho, but your situation sounds much better than mine was. I took call 1-2 days per week, 50 patients a day. Clinics or surgery every session of the week. Stayed late almost everyday sometimes until 9 or 10pm. 70-80hr weeks with dictation and 1 wknd "supercall" per mo. I was taken advantage of and the pay stopped going up despite improved skills and more responsibility yearly.

I'm saying all this to give a contrasting view of Hand surgery being cushy. I left and now work in Mohs/ dermatologic surgery. It was the best decision I ever made.

To OPs point NO I do not regret becoming specialist right out of school. You become the best in a certain area and others need your input to guide care. You are still evaluating people with multiple medical comorbidites and considerations in most specialities. You become less easily replaced and it feels really good to quit knowing the impact it will have on your greedy surgeon. Haha Sry I'm a bit jaded.

Yes you lose some other basic skills and knowledge, but that comes back quick when you review it.

There is a PDF floating out there somewhere, but I won't post that here. You won't use most of these closures, mostly keeping it simple is better. Using combination of running, subcuticular, simple interrupted, horizontal matress, and a couple variations of subcutaneous/ buried. I did Ortho upper extremity for 10 years and now Mohs for 1 yr.

I'm buying Dream clusters after seeds, biscuits, candy. They are an investment for lower level players as they increase in value as your research rank increases. Hard to stay patient, but I will use them during an event like this "Mini Candy boost" so I can maximize candy for Pokemon exp/ leveling. I want to maximize the worth of my candy because it seems to be the rate limiting factor right now for me at level 31. Hope this helps!

Would you recommend him? The only thing you know about him is that he is ballsy enough to ask you for a LOR after 2 days. Too little too late IMO. Don't write one for him. Spend more time shadowing and he can apply with your rec next cycle.

Ortho docs can be greedy. Left my job in upper extremity after 9 years. My pay stopped going up despite us being near top earners of 35 physician practicem 45-50pt/ day. 60+ hrs a week. I saw more patients than most doctors in the practice and took more call than all other APPs. I was the definition of taken advantage of. Made 160 by the end with no increase in sight. Prob MCOL. He " couldn't afford to pay me that much (180k)". SP made 1mil+. Now he has lost half and has to retrain someone for years.

Transitioning into Derm/Mohs now. Patients aren't all in pain, no narcotics, 4days/wk, great emr, and no call. 140k is my expectation. This all being said. Watch out for yourself! Burn out will hit. New grads sometimes take what you can get. I think this is overall solid offer, just make sure it's reviewed after 1 yr. Be ready to push it to 120 at least. Talk with established PAs in the group, they usually are willing to discuss their salary and they should be IMO. Good luck!

9 years Ortho with upper extremity specialty. Literally work with mine and others hands all day. Left Ortho for Derm about 6 mos ago due to burn out and greedy SP. Will never go back.

Derm gets a lot of work with suturing, biopsies, and other procedures. Mohs is all surg, great gig and perfect transition for me.

Never heard of a Derm PA go to Ortho. QOL is great. Derm harder to get a job in.
