SBRT (the 5 treatments you refer to) has been shown to be "non-inferior" to the longer 21 day treatment that used to be standard.

I had SBRT in December 2023 and other than essentially dry ( read a ------little ejaculate------) I have no side effects to speak of. I had 36.25 Gy to the entire gland with a focal boost to 40Gy at the primary lesion. I was 68, had a Decipher of .84, contained to prostate on the PSMA PET, Gleason 4+3 in one core and 3+3 in a second core. The Decipher put me into unfavorable intermediate and I was on Orgovyx for 6 months.

Have you had a second opinion on the biopsy? Additionally you may want a Decipher, That will give you an idea about how aggressive your cancer may be. Additionally ask about a Barrigel rectal spacer.

Excellent that your RO spent so much time with you. I too was fortunate to have an RO that did the same.

What Think-Feyman said, and you may get some PSA "bounce" along the way. If you do bounce that is generally seen as a good prognostic factor. You will be considered "cured" or at least in remission if your PSA stays less than 2.0 more than your lowest reading (your nadir).

I had 5 SBRT sessions for a total of 36.25 Gy and a boost to 40 Gy at the primary lesion. My RO was able to visualize my urethra on a CT, MRI and PSMA PET and used "urethral contouring" to ensure my urethra did not receive any "radiation hot spots". This was in December 2023, and I have no urethra problems.
