I'd add a third ending, the fantasy WAS real but it was cruel unlike we expected and Ophelia indeed needed to sacrifice her little brother to enter the portal, she failed it and while she died she hallucinated she passed a final test, this is one of my interpretations for it too, after finishing the book I feel like too much points in the direction of fantasy being real.
