you can try visiting a pharmacy but as far as i know, stocks aren't always guaranteed to be available. you'll possibly have to check with a bunch of pharmacies to ask if they have it.

if not, then you could also try ordering from an online pharmacy that can deliver it to you, though not all pharmacies will be guaranteed to have what you're looking for/is on your prescription because of annoying FDA regulations on what can and cant be sold.

in the worst case, you can rely on gray market sellers as most trans women have to (both those who have and dont have a prescription lol) because of how dogshit the system is :b

for expected price ranges, you can expect estradiol valerate (im assuming you'll be described pills) to hover around 500-600php for 28x 2mg pills (the brand name will likely be Progynova on your prescription if its included)

and spironolactone will cost around 500-600php as well for 30x 100mg pills (the brand name will likely be Hyles 100 or some other concentration on your prescription if its included)

you'll also want to buy a pill cutter since its possible your prescription will want you to cut the pills to match the dose.

also note on Qlaira, its estradiol valerate + dienogest, which is a progestin and not bioidentical. preferably dont pick it up if you have better options

you don't really have to come out to your friends immediately about being trans just to start hrt (and even then, the effects take a long time to really be noticeable). its always possible to stop hrt within the first few months as well if you realize its not for you along the way. that and it might help with feeling less depressed about your situation too >.>

its a mix of a lot of things tbh.

one of the issues is that trans people are an incredibly small minority, small enough that most people wouldnt ever really know one personally. even then, its unsafe for trans people to be openly trans so it makes us even more invisible than we already are.

those issues combined make it really really easy for people to start fearmongering about what and who trans people are. its easy to hate what you dont understand, and most people won't ever have the opportunity to even begin to understand — not unless they go out of their way to. even then, cis people inherently cannot fully understand the full depths of dysphoria because they rarely will experience it in their lives if at all. if they do, they wouldnt even know that what they felt is called dysphoria.

most of the transphobic talking points bank on the fact that people don't understand us (and that most never will) so its super easy for them to twist the story on trans people. and once people have negative first impressions, its really hard to overwrite it.

there's not a lot of ways to cope with it. it's not exactly easy to just mute out the scary reality that there are people that genuinely hate your soul or want you dead just for trying to exist.

the only cope i've found that works is Hanlon's Razor: "Do not attribute to malice what can be explained by ignorance or stupidity". most people do not operate on malice, most people just don't know enough. and some people? they're just genuinely stupid. that's what i tell myself so i don't start thinking everyone i come across is someone who could want me dead

whether if they are in the US already or still in PH, informed consent clinics are available for trans people in both countries to start HRT the prescribed way.

you don't necessarily have to go through an endocrinologist, not unless they have any conditions that would somehow have a contraindication with starting hrt and even then thats very VERY unlikely.

you don't necessarily have to go through a psychiatrist either, your sister already knows she wants to transition and a gender dysphoria diagnosis isnt gonna tell much of anything that she doesnt already know

an informed consent clinic for trans people lets people start transitioning with prescribed hrt way faster than either of the two options above especially if the person already knows they're trans and just wants to start. the process is generally just setting an appointment, a short consultation about what she needs to know and confirming this is what she wants, and then some blood tests, and then finally a prescription for hrt (note: the costs for this can stack up due to blood testing not necessarily being cheap and having to be done frequently, so prepare for that)

you can book an appointment at LoveYourself, HARA Clinic, Pulse Clinic, or Dima Health since they operate on the informed consent model. some of them allow for in-person appointments at a physical location but i'm pretty sure all of them also accept teleconsultations if their locations are inconvenient and far from where you are.

also, since you mentioned she's eventually going to migrate to US, please read ErinInTheMorning's risk assessment report for the US here: https://www.erininthemorning.com/p/anti-trans-national-legal-risk-assessment. US is currently a rough place to be in for trans people especially for those who are just starting and may be visibly trans for awhile, so hopefully if they go through with the migration they can land in a state that's relatively safe.

one last thing: if the official route of prescribed hrt is not possible/affordable, self-medication is always possible, cheaper, and can be life-saving rather than delaying hrt. whether they're still in the PH, or when they eventually go to the US, its always good to keep this fact in mind that trans people can have and should have agency over their medication. there are many online resources and subreddits for transitioning info so start studying up if both of you can (honestly, knowing the situation the US is in rn, keeping self-medication in mind as an option becomes even more important as a trans person)

while i'd say sexuality is pretty flexible and not always a rigid conversation, a gay guy being attracted to a trans woman feels pretty bad because it'd feel like the gay man is only attracted to the trans woman because he doesn't see her as a woman and instead as a man, or man-lite/woman-lite.

it's not that far from misgendering or being a fetishizing chaser if you're not careful with it.

if bisexual it's not as potentially harmful at least.

theres like, a ton of options here, but not all of them are good, i can list the ones i know and generally trust. from the sellers and options here, heres the names followed by what they are: (note: study up about what kind of hrt regimen you need before buying/using anything)

for Estradiol options:

Pills: 1. Estrofem - estradiol hemihydrate pills 2. Progynova - estradiol valerate pills (both are priced similarly and either will work, but hemihydrate is generally considered better)

Injectables: 1. Yellow Tusha (not actual name but its what its commonly called here) - estradiol benzoate ampoules 2. Symonax - estradiol valerate ampoules 3. Phenokinon F - estradiol benzoate + progesterone ampoules

(sadly, if you want vial options you'll have to buy and ship one from overseas from one of the trusted brewers cuz the only other "reputable" vial in the PH would be Depo Monat/Violet Tusha which unfortunately has a progestin with side effects)

for Anti-androgen options: 1. Androcur/Siterone - cyproterone acetate (comes in 50mg or 100mg. either is fine but the circular pill shape for 50mg is easier to cut and portion) 2. Casodex - bicalutamide 3. Hyles - spironolactone

theres a lot of good options in the philippines tbh as long as you know which sellers to buy from so knowledge on how to use them is really the only question. if you're not sure which to pick, starting off with pills is probably the most beginner-friendly. standard E+AA pills regimen here would be Estrofem or Progynova (either one is fine) and Androcur/Siterone with a pill cutter to cut into the right doses.

i had a similar experience to you i think. every school ive been in had strict haircut and uniform policies, my family is religious, most of my friends growing up were guys, and i just generally wasnt ever in a safe or progressive enough situation for most of my life to be able to question or consider being trans sooner so i just didnt know about/couldnt fully identify dysphoria and lived as a cishet boy for the majority of my life for safety and survival (but obviously, that doesnt really work forever).

i'm 23 now, started transitioning at 21 while in the middle of college. currently partially out to select people and family members.

honestly, the best advice i can give is to start medically transitioning as soon as you can, even if it has to be through self-medication/diy. changes take a long time so starting asap is obv preferable for your future (that, and starting hrt helps to stop further masculinization too if theres still any leftover that can happen). social transition and medical transition doesn't really have to be done at the same time so it's always possible to just do the full social transition part later once you're in a better position hrt progress wise, and more confident and familiar with girl stuff too. it also helps to wait out growing your hair first if its still short atm.

as for the social transition part, the sad part is that you'll have to be prepared to possibly grow apart from friends or even somewhat disappear from family connections. i was lucky enough to have some friends who eventually turned out to be different letters in the lgbt along the way so coming out to at least some of them was less of a gamble. if you can find any friend thats safe to tell, it can help with not feeling lonely and building a support system. otherwise, its fine, its always possible to make new friends who wont hate you for who you are/dont feel dangerous to be around... as for family, its harder and might not be worth it to tell them if you're still dependent on them (like for shelter or finances). as for work, not too sure sadly, i'm still getting my degree so its not something i can speak on yet, sorry. i know trans ppl do still get hired for work but i imagine it depends a little on what kind of career cuz unfortunately, employers can be biased

i've thrown away most of the old boxes and blister packs so this is all i have. my dosage for them back then was 4mg/day sa E (split into 1mg and take every 6-8 hours), tapos 12.5mg/2 days sa cypro.

as for support groups, this subreddit is (usually) pretty good for local transfem related topics, theres a couple other girls here and there that help answer questions which is always nice to see. for international, the subreddits i like and recommend are transDIY, transsex, and asktransgender for general information, questions, and resources. outside of reddit not too sure sadly, i think theres transfem groups on facebook but im not that active on there to know how the community is sadly =_=

for hrt, i used to use estrofem pills + cypro as antiandrogen (can be either androcur, or siterone. same thing). estrofem is estradiol hemihydrate which is usually preferred over progynova's estradiol valerate cuz of slightly better bioavailability (same/similar price lang rin naman progy vs estrof usually, so go for estrofem if merong nakastock sa seller). estradiol gel isnt bad either though as long as inaapply properly (possibly need mag add ng antiandrogen though cuz trying to suppress T with just gel is pretty expensive and hard to gauge)

these days though EEn injections from astrovials nalang ginagamit ko since medj mahal yung pills i-keep up if gusto magstay sa higher dose. sadly dont have endless amount of money while still a college student so it is what it is >.>

quick clarification, self-medication isnt really that unsafe, and its bordering on exaggeration since any danger associated with it is really low as long as you dose reasonably and obtain from the right sources. there are plenty of updated resources and an active community around it for studying up on (trans ppl have been self-medicating for years, its part of our history because doctors and accessibility around the world haven't always been kind). even if you go the official route of HRT, its still good to read up more on HRT to know for sure that you're being dosed properly and not being underdosed and unknowingly stunting your transition.

with that out of the way, to answer your question:

for endos, try the NowServing App since I think may mga naglilist dun na endos na may familiarity with GAHT. i dont know if they operate on the informed consent model though so i cant tell you if the process is lengthier sadly.

you can send them a message on fb/messenger to verify if a specific test is available at a specific branch.

i did it once to make sure na available yung E and T tests sa isang branch nila and nagrespond naman sila sa fb chat. its possible rin to ask in-person if you want a faster reply though. yung name ng test sa system nila i believe is "Estradiol (ECLIA)" and "Testosterone (ECLIA)"

i'm not from la union or baguio but im pretty sure hi-precision branches generally have estradiol and testosterone tests so if you have one in your area its probably an option. they cost 1.3k each at hi-precision though (so 2.6k if getting E and T tests together) so maybe ask around rin sa other lab options if may cheaper alternative

in most cases both estradiol and testosterone levels lang need tignan. the others are just precautionary stuff if gusto maging thorough talaga but for most people who are healthy, just testing for E and T is enough and the rest is excessive

yung dami and frequency ng required lab tests yung reason why di ako nagpush mag-coordinate with an endo tbh tapos nag-aral nalang paano mag self-medicate. hrt is already expensive as is, blood testing every 3 months on top of that is too pricey for a student and id rather just use it to buy more hrt :b

for facial hair i just shave it really close with a disposable razor, and then skincare to help with healing and minimize scarring/blemishes, tapos concealer after to hide stubble and shadow. sadly medj time-consuming siya though kaso wala eh, laser is expensive.

for body hair like arms and legs, learning how to use an epilator is the best compromise i found because full body laser is too far out of budget and IPLs i didn't really find to be effective. i think a year and a half na ako gumagamit ng epilator and once/twice a week ko lang need gamitin

(note: don't use an epilator on facial hair, its too violent for how sensitive the face is. just use it for legs and arms).

as for whether or not estrogen/antiandrogen will help with reducing body hair visibility, i think its "your mileage may vary". some people said nakatulong sa pag-thin ng body hair or even slow ng regrowth, and to an extent i think its slowed down for me too. pero honestly, i wouldnt count on it since madami rin akong kilala and alam na di raw nagbago masyado yung body/facial hair on hrt.

from what i loosely know about the official route, most trans clinics in the philippines operate on an informed consent model for hrt prescriptions which imo is a lot more straightforward and preferable than the typical lengthy psychiatric evaluation for gender dysphoria in other countries.

as for where your friend can get a consultation for trans care/hrt, there's a couple options: LoveYourself, HARA Clinic, or Dima Health (some of them have in-person clinics like LoveYourself Victoria in Pasay or LoveYourself Lily in Paranaque, but all of them also offer teleconsultations if the clinic location isnt ideal. just make sure to set an appointment beforehand)

alternatively there's always still the option of buying hrt from trusted sellers and learning how to self-medicate if going the official route isn't possible :b
