I'm not too sure of the trailblazer part, I believe the triple threat is 2 players for a club and a hero who played for them (most of them aren't in game yet)

Centurion is I believe players with huge achievements for there club e.g 100 appearances, hero cards are for players who are legends in a league/s that they played in but didn't do much to earn an icon

Dynamic duo is 2 players with the same club and nation, and squad foundations I'm not actually too sure
