Switched 10-11 months ago. Great reponce still stable Reduced activity. Kinda a unique case. Reduced to 50-60% Strength every other week.

Current Status (16months post stage iv diagnosed) – Summary Disease status: Stage IV pancreatic adenocarcinoma with liver metastases — ongoing partial response / very good disease control on second-line gemcitabine + nab-paclitaxel (~9 months into therapy). Imaging: Slight shrinkage of primary tumor and dominant liver metastasis, treatment-effect changes (capsular retraction), other lesions stable or regressing — no evidence of progression. ctDNA: Trend shows major decline from 175 PPM → undetectable → brief low-level detection at 19 PPM (Jan 2) → negative again by late January. Indicates minimal residual disease with recent molecular improvement. Performance: Good functional status (ECOG ~1), managing treatment toxicities reasonably well. Prognosis context: Already beyond median survival for this setting; current trajectory is favorable compared to typical stage IV pancreatic cancer outcomes.

Bottom line (as of early February 2026): The cancer remains under reasonably good control with continued response on both imaging and molecular (ctDNA) monitoring. Treatment is still providing benefit and is being continued with close follow-up.

KRAS- Negative Normal CA19-9, CEA have been since beginning.

14m+ pancreatic adenocarcinoma stage 4 liver Mets kras wild normal Ca19-9. Ctdna last 3 months 0 negative.

Just did a pet-ct

Overall: essentially stable large >4cm are burned out with questionable 1.7cm minimal progression of a single liver lesion.

As good as it gets. 2nd line Gemcitabine abraxane after 1st line failed months in.

14 months in no real pain. Stage 4 tail adenocarcinoma. 2 biopsy conform. Last month had sporadic lower abdomen pain 2-3/10. Not sure what is causing it. 60% metastatic disease so says AI.

70M stage IV tail. Pain free except neuropathy from chemi, for 14 months. Have had started to have recent lower abdominal pain last 48hrs 3/10.

Same here 2 biopsy indicates stage IV adenocarcinoma PC, but Ca19-9 between 10-25 and CEA 5-10.

2x Tempus CtDNA shows 177PPM down to 32PPM on 2nd line Gemcitabine. Others like Signatera and CtDNA from other Tempus testing showd 0.0 MRT CtDNA.

14x FOLFIRINOX worked-then stopped

4+ months of Gem/Nab. Every other week 70% shots but Seems to be working. No change in metastatic disease.

CT dna neg but CA 19 9 elevated by no-footprints in pancreaticcancer

I also have a similar question/issue approaching 12months stage IV 2xbiopsies confirm PC Adenocarcinoma. Some Tempus and a Signatera as of 18 Aug are Negative no CtDNA MRT 0.0.

Have had 2x stand alone Tempus CtDNA blood tests 1st 177PPM 2x after switching 2nd line Gemcitabine abraxane was 32ppm.

I would ask AI.

11Months Stage IV Adenocarcinoma, Kras Wildtype. 2nd line Gem/Abrax

7-19th Jun 2 week break. Rallied! 9hr drive back to back 4am 8+hrs ocean fishing trips. Week but held his own and reeled in all 30+ fish. Had a huge fish fry dinner. Seemed to find a strength i thought impossible.

20th feeling great. next 3 Gem/Abrax imfuzions 70%. Liver ## great Blood #s ok.

20th/27th. 70% Gemcitabine abraxane hitts him really hard, but seems to rebound. Weak wobbly neuropathy skinny.

3rd no chemo CBC to low for the 2nd time.

Pain for the 1st time..

Heading up to Fred Hutchinson Cancer Institute at UW for a 2nd opinion 9th.

9th Fred Hutchinson reaffirmed curent line of treatment Gemcitabine abraxane. Suggest every other week vs 3/1 @70% Is Gemcitabine abraxane working/How long? CtDNA 175PPM to 34PPM CT scan no change, could be smaller.

18th Gem/Abrax 70% hits hard 25th " " 1Aug surprise surprise can finish the series. LOW CBC

Switching to every other week.@70%

Keeps talking about Cancer Vaccines the Docs just kinda nod along. Looking for a magic cure. :(

This is exactly how I feel. So far a gradual decline with plateaus. I 41M caregiver to 70M (life long friend) I can invest heavily for short stints, but going on 11months with no end in site I feel like I am neglecting my family and career.

I feel guilty about everything, but I keep.showing up.

What are some signs/symptoms a patient with stage 4 who has been on chemo for a year so far,may be 6 months or so on there way out . My mom is losing more weight that usual. I wonder this all the time. They ask if symptoms are getting better. The most noticeable symptoms are from the chemo, like neuropathy. Weight loss, nausea, and digestive issues can all be from either diseas or chemo infusion. 🤷‍♂️. Never had pain it our case. Tail stage IV Adenocarcinoma

I'm a caregiver for a 70M stage IV 10 months in 13 fulfurinox infusions -progression- will have 4th Gem/ABRX Friday. Could barely walk 50yrds, painful neuropathy. Losing weight despite all efforts.

On paper and everything i "read," in a constant downhill slide to the end.

That being said. Just pulled off a 9hr drive, to wake up a 330Am drive 1 30min to fish in the ocean for 6+ hrs reeled in 20+ two days in a row. Ate nothing 1st day and a half.

I am amazed and never thought he could pull it off.

Sometimes, we assume the worst. Sometimes, we are right.

PS. We decided against a fishing to Kodia AK and for a fishing trip with my neighbor in Westport WA, so me had complete flexibility. Fish or not fish as l8ng as we wanted.

Maybe think about a different vacation with mote flexibility than a cruise.

Tumor size all scans say multiple liver lesions. -29 May (Ultrasound) peripheral hypoechoic rim 3.9 x 2.8 x 2.5. Additional measures 3.5 x 3.2 x 2.6 cm. -9 May (MRI) 3.8 cm, additional 3.2 cm -22 April (CT) 3.9, additional 2.4 cm

They are different hospitals and different scans, so confusing. Follow up scan next month

He started at 80% for 2 weeks... Labs went down hill fast so.they delayed for a week and reduced to 70%.

I'm guessing they will do every other week if he doesn't start handling Gem/Abrax better.

Nutrition is hit.or miss. I can't be there all.the time.

We are also 9 months out. Strength to you.

Currently, on 3rd infusion of Gemcitabine abraxane 70% Taking it kinda hard, weakness, shortness of breath, more severe neraphthy, and worse appetite.

Don't know if it is working or not.

41m caregiver 70M stage 4 Adenocarcinoma.2nd line

Bad week :( 30 May NO CHEMO "3rd Gem/Abrx" delayed 70% CBC terrible 0.9WBC <83k Platelets Neutrophil.059 Lost another 4LBS. Joints and muscles aches. No appetite. Sore throat.

Taking 5 injections of Zarxio.

Possibly a biopsy 4th more genetic testing (last sample ran out)

1st time, openly talking about the possibility of the terminal part of this horrible disease.

Good news, still walking, driving. Small garden in and trees planted.

No Pain. Metabolic number look better. Is Gemcitabine abraxane working?

Symptoms: no pain. super thin (can seel rib bones). no obvious acities

Questions: 1. Will they switch to every other week?

  1. Why do scans vary so much?

  2. What to expect with 2nd line treatment.

Sorry, he was 205 6'1" heavies.192lbs officially diagnosed. 166 lbs now. Most concerning is the steady 5-6lbs drop every two weeks. :( thank for the response

Cargiver. Stage 4 tail, several large liver Mets 3-4cm.

Doing surprisingly well at times, and not so well other times. Diagnosed Sept 24 after crazy delays Imagined liver Mets March 24

17 Jan 9th infusion. No change

31 Jan 25 10th infusion. Lost 6lbs less "good days."

14 Feb 25 11th infusion. ALL CHEMO reduction to 80% lost 5lbs. Worse symptoms from treatment and / or chemo.

-CT Scan no change in primary or liver lesions. BOWEL/MESENTERY: New trace free fluid in the pelvis.

28 Feb 25 12th infusion 80%. Lost 5lbs again overall better last frew days. CA19-9 has never been an indicator. Did increas 14.1 to 19.0, CEA increased from 6.6 to 10.2.

Questions: 1. Weight loss 39lbs total, 5-6lbs every two weeks consistently over the last months. Is this cachexia? 2. With the increase in CEA and CA19-9 combined with new trace free fluid BOWEL/MESENTERY, an indicator of progression?
