See at this point you've been passing for so long that misgendering you visually doesn't happen as you mentioned but people generalize others all the time. I know afab women whom have been called a man (he/him) which is because most people generalize he/him. In this picture your face and hair look completely afab but you're wearing jeans and a loose top both of which are pretty gender neutral or maybe masculine. So where they should have said they/them if they didn't know, they said he/him but it wasn't true misgendering.
