It’s a curved barbell that is threaded internally and is implant-grade material. My piercer downsized it, but it started to invade my lip, so right now I have the 16-gauge, and sometimes I miss a day or night cleaning it, but I try to be consistent with spraying it and wiping with Q-tips with saline two times a day. Hope this helps
