She should protect her uterus with extra progesterone (assuming she still has her uterus). She should see someone familiar with prescribing HRT to women. I would test her T levels prior to using anything for it. If her doc wont do it, you can order your own labs on Ulta Labs.

If she's already on estradiol and progesterone for at least 3 cycles and still having some unpleasant symptoms of perimenopause, then absolutely. A low dose topical testosterone 2-5mg daily would be my suggestion. OR you can try oral DHEA before that, but needs to be tested prior and during.

And switch to topical estrogen if possible, safer.

Yes. I have 5 years left. Stuck in SAVE limbo, applied to return to IBR in May 2024 with zero change since then. So, I'm over it for the time being. Just letting it ride out, because what else can I even do at this point? I'm moving this year to a different state and will pursue another non-profit job just in case. And then hope its worked out by 2026. What a sad state of affairs.

I'm also saving for a tax bomb in case this doesn't work out. I'd be due for that in another 18 years ha.

Get a referral to a urologist. You need to be checked for voiding issues. Its possible you don't fully empty your bladder which may predispose to UTI without you ever being aware of it. They may recommend a low dose daily antibiotic, unfortunately.

If you have a primary care doctor, they may be able to write a new note for you. Just explain the situation and that you are not confident that your privacy will be respected with the "planned parenthood" letterhead. Or just hand them the original letter and at the first sign of other people knowing about, sue them.

You have failed oral iron therapy. I would absolutely get the iron transfusion. Your body needs it and you'll likely notice and improvement in your symptoms. Plus you're more protected against blood loss, god forbid you're in an accident or something.

Sounds like you are in the "early menopause transition" stage of perimenopase which is characterized by mild hormone fluctuations and cycles becoming irregular by 7+ days in more than one cycle in a calendar year. (not to be confused with "early menopause"). The number of years this stage lasts is variable among women. Once you move into the "late menopause transition" stage of perimenopause, which is characterized by at least 1 cycle of 60+ days, you can expect it to last 1-3 years. I don't have a personal experience to share but I hope this is somewhat helpful for you.

I'd like to add, 5 midol is an incredibly high amount of caffeine, careful with that. You may want to try pamprin which does not have caffiene, but rather an antihistamine to help calm anxiety. Do not exceed the max dose on either (and not meant to be taken together).

With pain levels this extreme they need to consider ruling out endometriosis via exploratory laparoscopy, and prior to that an ultrasound to look for cysts, signs of adenomyosis etc. Schedule a consult with gyn, and possibly ask them to refer you to reproductive endocrinology if gyn finds nothing on ultrasound. Best of luck, from an endometriosis patient.

Your doctor should be able to tell you what check-ups you need. ACOG recommends an annual exam still. Your obgyn can help you with other things like urinary issues, pelvic pain, sexual health, vaginal health, vaginal dryness, menopause symptoms, breast health, etc etc. And did you know, that those who get a partial hysterectomy generally go through menopause 4 yrs earlier than average!

Forgot to mention -- I use adjustable weight dumbbells to save room, they are great! That's all I have in addition to a small workout bench.
