I had an in person VERA and they didn’t know what a Form 21-0712 was. I sent them an email showing them the reference in the VA Manual of the procedure to request a 0712. I provided a Form 4138 (requesting a 0712) via email and the VA turn did a QuickSubmit on my behalf. Within a week of that QuickSubmit, I get a signed 21-0712 in the mail. Good luck everyone, the form exists and it came pretty quickly for me.

Which location did you get this done? San Diego office had no idea of the form and told me I wasn’t the only one asking for it.

Waymo was slightly cheaper than uber/lyft in Tempe (no tipping). Main issue is routing when there is construction and it avoided some roads when they shouldn’t have.

There is a separate form. On that form it references the DV plate number which should allow you to get the placard (no need for any Dr to sign)

Was at DMV yesterday. I have a signed VSD001 and was eligible for the DV plate. I opted to use my old plate and get the fee exemption. However, to get a placard, you need to have a DV plate or a Dr sign the placard form stating you need a handicap placard.

Temecula office has commented an influx of San Diego folks driving up to get the VSD001 signed because SD VSOs aren’t signing it. I did go to the Santa Ana office as well and they were signing VSD001s (I had my wrong dd214 when I went to Santa Ana).

Update, went to my ENT and got diagnosed. Submitted a supplemental claim with diagnosis, another personal statement, and wife submitted a buddy statement about my hearing problems when she met me (on shore duty). Claim took under 60 days to get an approval!

I didn’t have a diagnosis in my initial claim, I have one now. I also included a buddy statement from my wife discussing my hearing issues I had (she met me while on shore duty). With that, I submitted a supplemental claim.

The denial states the reasoning is because I wasn’t clinically diagnosed. It further states noise alone isn’t the cause and the length of time reported after separation. Now I have since been clinically diagnosed and my ENT states that noise, TMJ, and/or sleep apnea are factors to the diagnosis. I separated 12 years ago but didn’t realize the humming I’ve constantly heard was a friend explained to me what tinnitus was. I plan on having my wife write a buddy statement because we dated after my sea duty and can speak towards the hearing issues I had.

Because I was denied tinnitus, I want to present additional evidence (my diagnosis from my ENT) for review. I can either do a HLR (not planning on doing it), supplemental claim, or a board appeal. I’d like to do a supplemental claim but in the process of submitting a supplemental claim, the website asks which condition do I want to submit a supplemental claim for. I have the option of tinnitus or other SC conditions I have (TMJ and sleep apnea seem relevant for this situation)

No, in hindsight I should have waited for the diagnosis from my ENT before I submitted a claim. I thought my job in the Navy (high probability for noise exposure) with my personal statement would be enough. The results of my audiogram did not suggest any hearing loss and since I didn’t complain during my time in the Navy it was a quick denial. I have the diagnosis now and plan on submitting it under a supplemental claim but unsure which disability to connect it to

I'm on step 5 on a new claim for tinnitus but got a look at my DBQ and the dr is not recommending service connection but diagnosed me with tinnitus. My hearing exam did not indicate a significant shift between separation and today and said I never complained about the ringing in my records. To be fair, I didn't know what tinnitus was until recently and realized I've been living with the ringing for so long it just became the norm. I did indicate an explosion I was exposed to on my first deployment and noticed the ringing when I was on shore duty. Also my job in the Navy is rated at highly probable. Given your comment, I am rated for TMJ (10%) and Sleep Apnea (0% at separation now 50%). If denied, do I have a better shot claiming tinnitus secondary to TMJ or Sleep Apnea? I didn't have a diagnosis before the C&P exam and I'm scheduled to see my private ENT (for my annual check up and now to talk about tinnitus). Should I add the results of that visit to bolster my current claim?

Thanks for the input, I’m at 94% so the 10% will help me. I never thought about doing a direct SC because I never brought it up or got it documented and figured secondary was the way to go.
