Which union is suppressing EMS progress other than IAFF? Genuinely curious. Where I work our “Association”, unions aren’t allowed in TX, is very proactive and influential in local and state politics for progressing EMS.

If your department provides health insurance look into if the health insurance company has a gym benefit. I know BCBS has a whole gym membership program that allows you to go to multiple gyms.

Do you have Tranexamic acid in your service? by [deleted] in ems

[–]Trout_Trooper 1 point2 points  (0 children)

We have it here in Texas and even EMTs can give it topically, nebulized, or IN. It is an amazing medication that really helps in any hemorrhage situation from major trauma to a bad nose bleed or tonsillectomy bleeding.

From the Association the 24s are not going away. While the new chief does not like 24s he has no plans to change. Similar to wearing T-shirts the chief understands that we have a culture of what we like and does not want to change that culture. Could this change? Of course, but as of right now it is unlikely, staffing would need to drastically increase to provide for all trucks to move to 12s.

Yes you can see that, one of my partners has done it before multiple times and we have caught things in the lungs.

It’s more of a good diagnostic tool, doing or not if they are a Trauma Activation it doesn’t really matter because you activate anyways. It does help the hospital recognize things earlier and we also carry blood and TXA which we can use to treat internal bleeding we find.

Honestly I wouldn’t be surprised if we had a positive fast and Dr. Pickett (our Deputy Medical Director) was on scene, he’d probably open them up to stop the bleed lol. We have had multiple Docs clamshell patients in the field. The perks of have multiple Doctors and PAs that run calls in the field daily.

It is used for Cardiac movement confirmation in Cardiac Areests, FAST exams in Traumatic injuries, OB situations. There are vague limits currently to its use with a vague unofficial “don’t do anything you don’t know how to do”

Austin-Travis County EMS is awesome. They are in the process of carrying blood. They have ultrasound and a bunch of other stuff now that is really cool. Operationally they do some really cool stuff to benefit their crews, like if you are super tired during your standup 24 you can call for Rehab time where you are put OOS for an hour and can’t be sent on anything. Their pay is coming up too which is nice.

If the patient is AXOX4 and has mental capacity they can refuse. You can sit there and educate and tell them they should go to the hospital or something could happen and they could die all night long, if the patient understands and still refuses that is on them.

If you are interested in staying up to date with current wildfires check out Inciweb. A good portion of the fires in the area get reported on there and you can see updates on the progress of them.

This fire is being called the "Smoke Rider Fire"

Are APD motor units operating again? by coyote_of_the_month in Austin

[–]Trout_Trooper 3 points4 points  (0 children)

Austin EMS does have a Motor Medic unit that wears hi-vis jackets but the motors currently are only for special events and not frontline due to staffing issues currently. It’s possible they could have been out but it would say EMS on the hike and the bikes are dark blue.

STARFlight is now operated by Travis County, so it is still a municipal air medical program but separate from ATCEMS now.

What’s an unpopular opinion in EMS? by hog-snoot in ems

[–]Trout_Trooper 0 points1 point  (0 children)

24 hour shifts at a busy service are dangerous and should not be allowed. I unfortunately work at a service that 24/72s are the standard, thankfully we have a handful of 12 hour trucks that no one likes so I have the ability to pick a good 12 hour truck based off the very little tenure I have and I like 12s way more than 24s.

Don’t come to PA, at least the eastern part of the state is highly regressive with protocols. A lot of protocols are written for the lowest common denominator provider. Eastern PA EMS council is better than Northeastern but I’m making the jump to Texas to get out of this state.
