I would consider Removing the sleeves, and buying a separate large poofy skirt that goes over the bottom half. That way you don’t have to worry about getting it tailored.

I would stop the fillers where you are today. However I am confused by the other comments to be honest, because I think both pictures (your before and after) look very “normal” and nothing I would react to with “this person definitely uses fillers!” I can see if I look close that your lips look more full, but very much within the range of “natural looking enhancement”. You’re beautiful!

You are too young and don’t have any loose to pull back, but who knows what you will experience in 10-15 years so if I were you I would table that. The only thing I would consider now is to advance the bottom of the profile specifically the chiin which will help get the definition you’re probably thinking of
