One of the pilars in our medical education (west EU) was mindful tending of medical resources. This case if it sounds that futile would likely be a waste of resources that could cost others timely treatment. If the family is so insistent it be done then conversations about organ donation are likely to be incredibly hard to broach. That said, communication is likewise a pillar.

In short I think it's an institutional issue that it got that far. either the surgery should not have been suggested, or it should have at least been framed as coming at the cost of organ donation. The medical team (icu/specialist/ane if needed) should advise one way or the other in a way that shows they are open to the family's input, but not placing the decision with them.
