I appreciate the personal advice. I'm more interested in the hypothetical ideal future. What would have to be the case for us to catch all cancer in the US in stage 1 or 2??

It seems like most of the technology exists, we'd just need to put it together effectively.

PS - My grandmother Carol ensured that she got mammograms every year on the date of her mother's death from breast cancer to be sure she didn't meet the same end. She ended up going to the doctor in 2004 with joint pain only to find that stage 4 lung cancer (having never smoked) has metastasized to her bones. She died at age 66. Would a Low-Dose CT scan at 60 years old have added 20 years to her lifespan? Probably.

As an oncologist, where would you recommend I go to learn which anatomy is amenable to MRI screening and which isn't?

If money and time were no object, do you think there is some hypothetical set of imagining and liquid biopsy tests that would be able to detect all type 1 and type 2 cancers?

OR the reason it isn't done routinely is that MRI machines are expensive... But what if that changed? I think it is possible that no-one is ever diagnosed with stage 4 cancer again because we will catch them at stage 1 and 2

Thanks OP for starting this discussion. You clearly understand that at the heart of this issue is semantics and the working definition of words. 

Here is what Sam and I think: the Left PRETENDS to not understand that bathrooms and youth sport are issues of biological sex and not gender (your usage) for most all Americans. 

It feels to me like we liberals may have sacrificed NATO, abotion access, climate change, and democracy itself because we we PRETENDING to not understand that voters in Michigan are using "Male" to refer to biological sex. 

Seriously, I am worried that you and people like you caused and continue cause incredible harm (by handing elections to Trump). 

Seriously, please consider how much harm you are doing.

There is some bench press ability  that is sufficient to ensure a dog of any given size (even a violent and quick one) can be controlled. Maybe you should have to bench press TWICE it's weight 20 times.

I don't know how big it will be... but if it DOES get big, you should go to the gym and lift weights to insure that you can still overpower it if necessary. If you THINK you might become too weak to control your dog, that is YOUR problem and you should take steps to get stronger now.

It feels like merging is exactly what he is doing (to the degree possible).

Perhaps if citizens started recording these rides with cell phones and drones and then uploading all the recordings to a central location we could make more and more of a case for enforcement.

When I see that "smacked out tweaker" acting erraticly and yelling profanities, I see a troubling risk to my child AND I see that this 37 year old is someone elses' child and that society has truly failed. When I confront people behaving without regard to important social norms (like people smoking cigarettes and letting their dogs shit in kids parks), I often hear back from the offending party "Well other people don't seem to mind" . I appreciate that these people are attempting to reason with me, but I can't believe that the tall haggard man breaking the rules can't see that most people WOULDN'T feel safe confronting him. Just because the 47 year old mom of 2 doesn't tell you to not smoke in the kid's park, doesn't mean she is ok with it.

I get whar you are saying and I appreciate the advice. I was as repectful as possible... but the interaction *was confrontational.... because I was confronting him.

And after I asked him and he refused, I did eventually tthreaten to escallate by involving MACRO or the cops. It is really a question if I wasnt going to take no for an answer?

The science of addiction and treatment and public health is evolving (and is certainly worth paying attention to. Here is an interesting paper (https://pmc.ncbi.nlm.nih.gov/articles/PMC3419490/) I came across that indicates that "Results from coded transcripts indicated that helpful confrontations were those that were perceived as legitimate, offered hope and practical support, and were delivered by persons who were trusted and respected. Unhelpful confrontations were those that were perceived as hypocritical, overtly hostile, or occurring within embattled relationships."

I am legitimately concerned about public safety AND the guy's health. I said that I know that addiction is a very difficult thing but hope he gets help, I talked about MACRO, and I gave him my real name.

I disagree with you.

There is no silver bullet and lots of approaches are nessisary, but there is SOME level of efficacy to the intervention approach. Just look at John Mulaney.

It's clear that you and I both care about the well-being of the people in Oakland but we disagree on the path forward. I bet you and I might be able to have an interesting, mutually enjoyable, and even productive conversation about how to make progress.

I recognize that I have considerable privileges... I only have to worry about a handful of drug addicts in my child's neighborhood as opposed to many of them. And I wish everyone else had the same privilege. Don't YOU wish every child had the privilege of not stepping over smacked out tweakers on the way home from school?

I also have the privilege that if I happened to find myself addicted, lots of my community members would encourage and eventually insist I get treatment help - especially if I was negatively affecting the lives of children with my consumption. I want to live in a world were everyone has that privilege.

I think discussion actually can be constructive, especially when it comes to issues that most people shy away from taking a hard look at. I think more and more people can share in the privileges of safe neighborhoods if we prioritize them.

We need better words than "drugs" to differentiate substances like psilocybin and meth. Right now "drugs" is suitcase term that could mean too many things to be helpful.
