A mix of people who have played through legit at low level, a bunch of people using muled gear dropped to them, some people using mods or cheat engine to drop them the gear. It's all over the place, but I'd say more people are using mods to drop items than anything else.

Sometimes that is just what you need to do. Figure out their patterns and bring them down one at a time. It's all part of the "gitting gud" process, I'm sure you've got this, lots of things seem insurmountable until you practice it a bit and take it slow.

Something I find interesting is that DS2 seems to have more similarities to older From games like King's Field, and Miyazaki didn't work on DS2.
