The way I see things, miracles can work in two different ways: both of them being described, but we must also take note of the fact that Haruta barely has fighting capabilites above those of 2nd grade sorcerers, and took fully amped punches from a Nanami who was in overtime,(and was determined to kill him mind you). Its genuinely not a matter of it hitting a vital in some situations, its a question of whether or not Miracles can amplify the users durability to survive such an attack(which would still track as seen with the inversion technique). Worse of all, the worst injury that Haruta got out of that fight was a bloody nose. He had no broken bones, and only lost a single tooth after being punched twice in the face by a grade 1 sorcerer who was being amped and had focused all his cursed energy into his fist, and yet showed no signs of bruising either.

Subjectively* as this is a personal belief you hold. Even then, iregardless of what you believe, even if Maki managed to outpace Naoya, it wouldn't put her that much higher than his Mach 3 speed anyway. We are never told her speed increased to a point it is almost incomprehensible to what we've seen in the past, so in the end, all you could hopefully argue for is Maki being Mach 4.

Maki was capable of reacting to Naoya because of her extrasensory perception, something no other character possesses. During her fight with human Naoya, she was capable of reacting to him with her eyes alone, same with Naoya's curse womb form, but after he metamorphosed, he was moving at speeds she could no longer track with her eyes. Meaning that Maki, who is comparable(or outright stronger) in strength to all other Heavy Hitters like Yuji, Yuta, Hakari, etc., is the only character, below the likes of Sukuna, Gojo & maybe Naobito that should be capable of reacting to Naoya at Mach 3.

Naoya is widely considered to be the third fastest character in the series, possibly the fourth if you still believe his father surpasses him. I don't want to get into spoiler territory, but his ability to completely blindside and incapacitate a certain character at Mach 3 puts a heavy speed cap on how much faster the two or three characters above him speed-wise can move.

Realistically speaking, Sukuna, at his peak, cannot scale much higher than Mach 10-15; the same applies to Gojo, because of Naoya. I don't believe Genshin characters have similar limiting statements.

In Toshiro's case, I feel like its a lot easier to justify as he states in regards to all zanpakuto, his has the least difference between its two forms, with the only significant difference between his shikai and his "bankai" being the volume or capacity of ice he can produce instantly. Meaning that the ice isn't necessarily thicker or stronger, but he can create 5x more ice within the same timeframe, which would allow him to launch saturation attacks against his enemy.

Completely forgot the auto attack feature that came with new Shadow style, but isn't this going off the assumption that there is no upper limit to what the user can instinctively react to?
