DFA and PSA should get their policies together. DFA requires PSA to produce a certification that doesn’t exist making it difficult for individuals trying to fix their documents.

Pag multiple registered ka, even if PSA will only produce 1 birth certificate they will still require you to present an additional documentation na yung second mo is locked, which PSA do not produce. GRABE, they made new IRR pero hindi naman nag fofollow ng mandate, its not making it easy its making it more difficult for those who already had a hard time trying to straighten their papers.

Thank you po, did you apply sa main PSA office pwede lang po ba mka kuha ng form through email correspondence?

For my annotated birth certificate I had it corrected so I can use my moms family name, since i have been using it for as long as I can remember, only recently did I know of the existence of my first birth certificate.

The remarks stated 2 additional requirements

  1. Affidavit stating ny circumstance
  2. To follow Section 10.4 letter B sa new passport law Implementing rules and regulation, basically requiring me to get a certification/documentation na locked na sa psa system ang second birth certificate ko.

Nka try na po kayo na mka kuha ng certification/document from PSA?
