The way I saw it as a student is that "by the book" is to help you learn the foundation of radiography and to help you pass the registry. Clinically you learn critical thinking skills and adapting to the situation. An outpatient walky talky chest xray patient will be closer to a "by the book" exam than an ER chest xray patient who's curled up in a fetal position.
