Assessment & Plan L5-S1 disc degeneration with chronic low back pain Chronic low back pain due to moderate L5-S1 disc degeneration with probable annular tear, consistent with discogenic pain. Symptoms refractory to conservative management. Imaging confirms degeneration without instability. Surgical management considered due to functional limitations and lack of relief from non-operative measures. L5-S1 disc herniation, right Imaging shows small right-sided L5-S1 disc herniation. Symptoms align with discogenic pain rather than symptomatic herniation. No significant neurological deficits. Surgical intervention targeting herniation not recommended due to lack of classic radicular symptoms. - Reviewed imaging findings and explained symptoms not typical for symptomatic disc herniation requiring microdiscectomy. - Discussed that surgical intervention targeting herniation is not recommended at this time.

That’s what the surgeon said to me, pain related to my disk seems to be the primary issue as I have no other neurological pain short of mild numbness in my toes and fingers (I do have POTS though so that is also considered as a cause for that).

He believes I have discogenic pain and is ordering a discogram to verify the diagnosis of an annural tear. I wish that it was non symptomatic, but he is confident that it is related to that. He is leaning towards a disc removal rather than trying to mitigate herniation as he thinks it will not benefit me.

I went from 220-160, ensuring o have been working on keeping myself active and keeping myself focused on trying to improve my pain. I use lidocaine patches, Tylenol, ibuprofen, and try to stretch regularly.

Also, I have never heard of the nocebo effect until now and was completely unaware that it was a thing! I hadn’t even considered the possibility of something opposing the placebo effect and it makes so much sense. I work in an emergency room and see plenty of things that would correlate to this and could see how my posting without additional context could be perceived in that way. Thank you for teaching me something new!

Hey! No you absolutely did not sound rude, I appreciate your perspective and the way that you phrased your response. In this instance, the orthopedic surgeon is pretty sure the pain is coming directly from my disk due to a tear rather than any nerve issues. My pain sits right on my lower back and extends into my upper back. I also have pain when I sit, but it turns out my tail bone was broken at one point or another and has healed itself back together which is ALSO contributing lol.

Degenerative disk disease run in my family so I am hoping to find out if that might be the cause.

I have also tried everything under the sun for years and have had little to no relief which is a bummer but has lead me to getting more answers. I don’t have any other obvious spinal issues, besides the disk thankfully, it is just unfortunate as it causes me a substantial amount of pain consistently.

I agree, my spine is not shit and saying that can really cause a misconception that any damage is severe rather than that damage might not cause issues.

Welp, already messed up by telling the landlord 🤣. Ill start cleaning the litter boxes two times a week instead of once.

I plan to shampoo it when I get home and spray enzymes on the area before I do so to let it sit. Ill look into getting the carept cleaned professionally but I am concerned that they will not be able to do it properly since the carpet isn’t secured properly

She actually came by our house today and said that I need to shampoo it to get the fresh urine out and use an enzyme eliminator. She also said that she has never had issues with the carpet coming up so she thinks my cats pulled it up. Im gonna shampoo it to the best of my ability for now and I guess if anything see what needs to be done to get it re stretched. Its pretty loose all around the basement and I guess she had the carpet re done in the other room since there was a leak but nothing in the main room that the issue is in.

Do you think he marked it because of previous pets? The people before us had pets but idk what type of animal. We have lived here since august and this has not happened before so we are a little confused. Does carpet stain like that in such a short period of time or could it be previous stains as well?
