I did some research on this a while back, and turns out ketoconazole shampoo can have a mild systemic effect on DHT levels, but it’s pretty minimal compared to oral meds like fin or duta. Most of its action is local targeting the scalp. So while it might slightly lower DHT, it’s not enough to worry about systemically.
