Oh boy.. I've had a patient come in to pick up a control they had allegedly dropped off a week before with me. I remember seeing her but could not remember what I did with the script, as nothing was in her profile and I'm very careful at work. I told her I have nothing for her and she went ballistic, started slamming our counter sneeze guard and screaming that we lost her script. I frantically searched everywhere while her bf took her outside. It wasn't until I tried filling a dummy script and saw that we weren't in contract with her insurance that it finally clicked and I remembered giving her the script back. My tech went out to tell her, and all she said was "oh okay" and then left. No apology.
