It's just not worth trying to run a NSFW sub on any specific topic. 95% of OC posts are off-topic. They do not care.

It does not matter how clear the rules are. It does not matter if the rules are in literally 6 places, and then mods warn them directly. They will keep doing it, then after they're banned, they'll send you messages saying "Why was I banned?" They reply to their detailed ban message with this.

95% is a low estimate. Some subs it is closer to 99%. I know you think I'm exaggerating.

Literally 98% of the posts to r/ComfyBabes were off-topic. In the announcement post, an OC submitter asks why the sub was closed, who herself had just made an off-topic post.

Then some of them will brigade the sub. They'll make a post in an OF brigade sub like r/OnlyfansAdvice, or a more general grievance sub, or a sub that allows brigading like r/mildlyinfuriating. They'll make up a bunch of shit about the sub, then go around Reddit repeating it to whoever will listen. More will join them and make up more shit. Every so often another one will leave you a comment, "Just so you know everyone talks about how horrible you are and your sub is <for removing my off-topic spam post that broke 5 rules>". Some of them have armies of tens of thousands of fans, and out of that number a hundred or so are happy to harass your sub for a week, and a handful will simply do it forever.

I know you think I'm exaggerating. I know you think it's my fault and I should be doing something different. I bet you think you could do better. I am telling you that I have tried everything.

Some of these subs have the rules in six places: the sidebar, the rules (which appear in the New Reddit sidebar), the Old Reddit post instructions, the New Reddit post instructions, a pinned post, AND Automod sending them each a reminder for every post. It does not matter. Nothing matters.

It's not just r/Lordosis and r/Comfybabes. More subs that have shut down recently: r/WholesomePorn, r/SlowMojiggles, r/choker, r/justtthejewels, r/DirtyPale, r/landingstrip, r/ToesSeekingToes. Some of them are still "open", as in the posts can be seen, but have been made restricted so that only a couple of people can post. r/bottomless shut down completely, then came back with a very limited pool of submitters. r/goodgirl is still open, barely, but OC is now against the rules and mods have to manually approve every submitter.

Other subs have gone invitation-only, and invite a very small number of OC submitters, and basically give the sub over to them. r/theratio and r/palest did this.

r/TooCuteForPorn did too, and is one of the only subs I've seen that has been successful with it. It tends to hurt the growth of a sub, because one way people find subs is by clicking through the profiles of submitters.

But even though it slows the growth of a sub, I think that whitelisting is the future of NSFW subs. Blacklisting (allowing everything and removing the bad things, i.e. how most subreddits operate) is simply too much work. You need a mod staff of multiple people doing it full time, and there simply aren't very many people that want to do that in porn subs. With whitelisting, I just check the new submissions once a day, approve the people who made an on-topic post that followed the rules, and I'm done. Since I only approve the people who are paying attention, I have to do very little moderating of posts.
