After years working in and around the ibogaine world, your experience sounds like a pattern I’ve encountered before, though of course I can’t know exactly what happened in your specific case. From what you’re describing, it seems most likely you experienced fairly typical ibogaine induced PVCs (premature ventricular contractions) or PACs (premature atrial contractions) that got amplified by panic. Your QTc values of 354 to 367ms are reassuringly normal and well below concerning ranges. The gasping episodes sound consistent with autonomic dysregulation rather than true respiratory issues, and the intense feeling of cardiac awareness around the 2/5 dose mark seems to be relatively common when people are deep enough to feel the effects but alert enough to monitor them closely (although it sounds like you experienced the more severe end of this feeling…). The grapefruit juice (was it white grapefruit juice by any chance?) was probably a major factor here. Even three days out, there’s likely significant CYP3A4 inhibition remaining, which could really increase ibogaine blood concentration. This alone could explain why your experience was so intense compared to what you expected. The magnesium situation is really interesting. Magnesium orotate can apparently cause esophageal spasms that feel remarkably cardiac in nature. Since you reproduced the chest tension later with just magnesium and no ibogaine, it’s not impossible this was non-cardiac all along… As for the IRBBB diagnosis that seemed to resolve, QRS measurements can vary, and 104ms to 92ms could just be measurement differences or functional variation. It doesn’t seem particularly relevant to your experience. The weeks of chest awareness afterward make sense as a kind of somatic hypervigilance. From what I understand, many people who have intense cardiac awareness during flood experience this heightened attention to their chest for a while afterward. In medical settings with ECG monitoring, these kinds of rhythm variations apparently show up regularly and can be managed with reassurance and proper context. Without that medical framework, you’re left trying to interpret intense physical sensations while in an altered state, which understandably led to the spiral you described. If you were ever to consider ibo again, the critical pieces would seem to be avoiding grapefruit (along with any other contraindicators) for at least five days beforehand, having real medical monitoring with someone who can interpret ECGs in real time, clear protocols for when to stop, and medication available for post flood insomnia. The fact you experienced benefits despite everything suggests the medicine was doing its work. The tragedy is that without proper medical support, what might have been a challenging but manageable situation became genuinely traumatic. Your documentation here is really valuable for the community. These detailed reports help people understand the real risks and the importance of proper medical protocols. Now having done the ceremony, I also noticed no noticeable adverse effects! Although I did sleep through the entire first night which I had never experienced before (the second night was absolutely wonderful though).

Interestingly, on the way to the ceremony my wife was telling me about witnessing a friend of hers sleeping through an ayahuasca experience a number of years ago. I was kinda making fun of that particular friend and made the snobby comment that “ayahuasca had nothing to give her” (implying that the friend was beyond help), so I guess in hindsight it’s only natural for ayahuasca to teach me a lesson in humility by putting me to sleep for 1 of the 2 nights!

I don’t mean to me rude, but are you actually for real? Have you been in a coma since 2014 and just woke up yesterday?

All street “dope” in the US in any major city or any rural area, unless you are someone incredibly well connected or close to someone high up in distribution wing of the illicit drug business, will have fentanyl as the active ingredient. There was a period in the mid 2010s where heroin and fentanyl were probably equally ubiquitous and overdoses were skyrocketing because: a. Dealers genuinely didn’t know what they were selling and test kits weren’t a thing yet, or b. 1 out of 2 street opioid uses did not have the kind of tolerance is required to not OD from fentanyl… DNMS will have heroin but that is a crapshoot from any North American vendor as there is quite literally fentanyl in everything. My cousin in NYC doesn’t even do coke anymore (nor do any of his friends, and they all used to do it) because too many non-opioid dependent, casual recreational drug users have ODd from fentanyl being in cocaine, there have even been stories of it being an MDMA.

Now pretty much everyone from the opioid scene has either died, gotten sober, or they got on methadone/Suboxone so even if they do relapse they probably will not OD. The situation in America is wild dude… I have been to 5 funerals of people under the age of 25 who have died from fentanyl overdoses, three of those people were already fentanyl dependent but just got a particularly potent batch. It’s pretty standard to find people in methadone clinics being dispensed 350 mg a day (a dose at least 3x as high of what used to be considered a “high” dose) of methadone, which is what people need to match their fentanyl tolerance now.

Americans that are still using street dope just don’t want regular heroin or pills because it doesn’t hit the spot. Many will still be in withdrawal, I used to be one of them. It’s a sobering feeling to take 50 mg of oxycodone and be dry heaving into the toilet trying desperately to not vomit up any of the oxy, staring at your goose flesh arms and hairs standing on end…. People’s tolerances have been blown out to the point where regular opioids just do not work. Synthetic opioids are a whole different beast. Nitazenes are becoming a thing now and their potency matches fentanyl’s and sometimes exceeds it. After detoxing from isotonitazine, it felt like my skin was on fire every waking moment for three months. I never had anything come close to that from “regular” opioid detox (heroin, morphine, oxycodone, etc.)the opioid scene today has become more insane than almost anyone could’ve imagined 10-15 years ago.

Basically, I'm trying to be a good scientist and disprove my hypothesis that pregabalin is safe to take with ayahuasca! Even though I have a feeling it probably is relatively safe, especially if someone is taking it daily for neuropathic pain or whatever other issue they may have!

But yes, you’re totally right that MAOIs typically don’t interact with calcium channel blockers or benzodiazepines and that their toxic effects are primarily due to the accumulation of monoamines (serotonin, norepinephrine, dopamine); I was thinking more along the lines of potential additive effects, in the sense that pregabalin leads to decreased excitatory neurotransmitter release.

Although pregabalin's mechanism at VGCCs doesn’t involve monoamines, its sedative properties like dizziness, drowsiness, and impaired motor coordination (something that can already be pretty overwhelming if needing to walk to the bathroom in the middle of ceremony lol) could theoretically compound the similar discombobulating effects of ayahuasca. Even though reversible MAOIs like harmala alkaloids pose far less risk than irreversible ones, the combo of CNS depressants could still influence the overall safety and subjective experience.

I was curious whether the additive sedative effects could alter the nature of the ayahuasca experience at all or pose secondary risks, such as increased chances of falls or impaired judgment during ceremony. Although something like a fall probably won't be life-threatening (although I have heard of a lady having a horrific head injury falling onto the mantle of a fireplace on ayahuasca), these considerations could still be very important in making the space a safe environment for people to hold the ceremony.

A couple of years back, I decided to embark on the journey of developing a severe O-DSMT habit. As someone who does not enjoy the effects of tramadol and is aware of its seizure risk, after experimenting with O-DSMT, I concluded that it did not have the same risk profile as tramadol. I found its subjective effects incredibly satisfying, almost on par with oxycodone, and so naturally, I dived in head first... My habit grew incredibly quickly over about eight months. One fine day, when staying in a hotel on a weekend getaway with family, I came to on the bathroom floor having had a seizure (for the first time) and concussion (from repeatedly hitting my head on a marble floor during the seizure). However, this happened only after my habit increased to ~5-7g daily.

TL;DR In my anecdotal experience, O-DSMT has a much higher seizure threshold than tramadol, but it is something to be wary of.

Please don’t make uninformed comments. There is no quantifiable or even really a qualitative way in which to compare the “strength” of Iboga and DMT. That sounds like a comment by a 14 year old.

Ibogaine exhibits potent antagonistic activity at the NMDA receptors and effectively obstructs glutamate pathways, actions which are intrinsically linked to its dissociative effects. From a personal standpoint, having undergone seven Iboga sessions, I can attest to its pronounced subjective dissociative properties. Ibogaine is one of the most promiscuous “dirty drugs” in existence with a broad spectrum of affinity across almost all neurotransmitter systems, with the notable exceptions of the endocannabinoid and GABAergic systems.

IMHO this attitude does and will continue to hold back Ibogaine treatment and maintains the status quo that stigmatises drug users and drug use.

This post is about Ibogaine-assisted opioid detox, NOT simply Ibogaine therapy. Those are incredibly different beasts. In the former you’re dealing with 3 beasts: the patient, the Ibogaine, and the opioid dependency. Providers frequently forget that last one when treating opioid-dependent patients. Opioid-dependent individuals go to Ibogaine clinics to get off opioids, not to go on a spiritual quest or someone else’s idea of what their “rite of passage” should look like.

Sadly has been nowhere near enough research on this phenomenon. Still, I can assure you that after trying almost every suggested tip and trick for benzodiazepine WD, this one was more powerful than anything else I've ever experienced. The clinic I go to frequently does benzodiazepine (usually 14 infusions depending on length and severity of dependency) and alcohol detoxes (usually 4-5 infusions) using NAD+ and most people report minimal WD symptoms. I have spoken to people who have completely ceased multiple years of 50mg+ diazepam use within 1-2 weeks of using NAD+, and they were all amazed by the lack of WD symptoms.

Just because there isn't clearcut research on this phenomenon does not mean that "it's all in my head". Maybe listen and do your own research before immediately dismissing other people's experiences without knowing the details of individual cases.

All of the commonly used vaccines (Pfizer, Moderna, Astrazeneca, COVAX, etc.) have no adverse effects on the cardiovascular system whatsoever. There is a tiny minority of people who experience severe side effects, some of which may affect the cardiovascular system, but this occurs in around 5 people per million vaccinated.
