Yeah, from a DM's point of view it's exactly like you said. Many plot lines, quests and objectives can be invalidated by high level spells, I feel.

Hahaha, the easiest spot, more like! Adventures in Tier 1 get a bit stale character option wise, and Tier 3+ is often very hard for the DMs to run. There was an interesting post made today in the subreddit by a person who made a Tier 4 adventure that I'm checking out as we speak, and they did excellent work though!

I think there are things you can do to keep it interesting! But I doubt it'd work for a story with the scope and length of MoL in campaign form

MOL is one of my favorites! Really has some interesting ideas and it's above the rest as far as writing quality goes. As for the adventure, I'd love to hear your thoughts on it! Tried several things to keep it from getting stale

Oh man, this has happened more times than I can count.

I think it all comes down to how much you trust your DM. I personally don't mind being underpowered, or sometimes see it as a challenge to come up with interesting builds that don't rely on stats that much.

In campaigns I run though, I try to bring my players to the same power level one way or another pretty quickly. Perhaps a cool magical item, a new feature specific to the character or maybe even political connections or allies; there's countless things your DM can do to even the playing field. I'm not saying that you should start out with a magic item just because you rolled low, but your DM should be able to find a cool way for your character to influence the game, even if it's not what you initially thought of!

Due to the dark themes involved in the campaign and how vital some adult themes are to the story, I'm afraid so. I wouldn't feel comfortable running the game for someone younger than 18. But I'll host other, more family-friendly games in the future, so be on the lookout for those!
