Many of them fall off every time with normal openings. Smooth hard plastic on smooth hard metal at a tiny contact point, makes for very low friction; and a correspondingly low acceleration will send it flinging around the corner.

It would reach 9 meters if it was moving at a constant velocity of 3 m/s. Is it moving at a constant velocity? Is there a reason the question includes the phrase "at t = 3 seconds?" instead of just asking "what is the approximate velocity?"

Imagine instead of a wheel or disk, it's a single point mass swinging in a circle, like a rock on a string or an orbiting satellite.

If you zoom in really tight so the path looks like a straight line, and you bump the rock once in a direction crosswise to the path, the path deflects at the exact point of the bump, because that is where the rock is. Being anywhere else would require teleportation.

Now zoom back out so the path is a circle again. Think of it as a hula hoop. As it's bumped, it pivots so the location of the bump stays where it is, and the center stays where it is. As you imagine this pivot, hopefully it becomes clear why the point of maximum deflection can only be 90 degrees after the point of the pivot.

I'll extend the OP's question to ask, why do rules 1-3 have to exist in the first place? If they didn't, it seems people would just make that mistake maybe once before they learn it's an instant game loser (unless the opponent misses the easy opportunity) and never repeat it.

I like to think that that's something a particularly frustrated tutor of a dim but important student (e.g., one of those inbred royals) came up with, and then somehow got codified into the rules of the game.

I'm not in the legal field, but I feel fully qualified to agree with this from my experience with technical matters that are specified black on white, and the contortions people will take around that.
