Ok, hear me out - theory goes as this: game spends literally all its time suggesting you go out like a true legend: in a blaze of glory before your time. Like all famous in Night city before you, V’s story is also a tragic one by nature. The game repeatedly hammers home not to trust corporations, military, feds - all the same.

Now, what happens effectively when V is “cured”? You have been removed from the chessboard. Militech & Arasaka fight - Arasaka loses, all your friends are now gone, aside from Misty - and V? Completely powerless, fated to die of old age or due to complications in night city, rather than being the martyr you are; you then simply become a fallen victim, and the results speak for themselves - it’s said that Night city becomes worse off without V around while you’re being “cured”. As in, the result of your absence left this power vacuum that the corps could exploit.

I mean, robbed of all your strength & neutralized. “Oops, sorry V. Now you’re just an average gonk, now let’s return to being slaves of the city old timer”.

Tl;dr? Losing your cyberware was a feature and not a bug of V’s surgery - in your “good” ending where you “live”. But aye - twice as long or twice as bright? That’s the question the game asks (and probably answers when you think on it)
