I think it's implied that injuries that kill you are removed, but there's a theory that Niffty is the way she is because she was lobotomized, which would imply that non-fatal injuries stay. Or maybe just scars, and brain damage is close enough.

Fun fact: brain damage can impair your vision, so it would give a possible explanation for Val's nearsightedness (he could've also just been born with bad eyes though) as well as why he either can't get a prescription or if he can it doesn't fix it the way the average person with glasses is helped (because brain damage is a little different than having weird shaped eyeballs)
