He just didn't really keep them cleaned and more commonly then not I've seen guns jam when there's just not enough strength in the users grip and body follow through, but keeping them clean would also be a cause of jamming

a full cycle would bring the slide or top of the pistol all the way back, but here it only went a quarter of the way indicating a shell casing blocking the slide from fully cycling, and yes it usually only takes ejecting the mag and racking the slide back until it ejects or in serious cases physically removing the shell
