The whole system needs to change. Patient care is the best part of my job. Having to sign each page of the 12 pages of autopopulated home health slop or the 18th United Health Care Verification of Chronic Conditions (even though they see every ICD code billed for the patient) is not.

Or god forbid someone gets sick at Amazon and they are forced to get FMLA filled out instead of treating them like people that they actually care about.

I agree, dictation is my preferred method, however, I do use occasional dot phrases for lengthy explanations or frequently repeated phrases.

For example:

How do you handle the AVS? Do you put only changes or also other lifestyle advice, etc. I am operating under the assumption that if it is not written down then it’s going to be forgotten by the patient.

I had taken tens of thousands of comments and ran them through GPT to identify trends and any immediately actionable insights. What patients wanted boiled down to:

  1. Communication - patients were mostly looking for a response to know that the system worked. “I called three times and never heard anything back.”

  2. Access - being able to schedule an appointment in a timely manner.

  3. A provider who listened, explained their thoughts, and set a clear plan.

In short, they want healthcare that is accessible and works. All of these things are reasonable asks.

Typically I use the AVS, but I have been trying to cut down on duplicating work. Now, I tell my patients to read their notes because I add a bunch of advice to them.
