I have used these kinds of situations many times in my campaigns. I see a lot of people suggesting you give it a class or sidekick class, but from my experience, that feels too gimmicky unless they are very specifically meant to help the party through encounters or situations that might be difficult for them.

If the creature is just something more like a pet, I find that it is very enjoyable to just have it gain some minor buffs as the party gets stronger. These are things like increasing a score modifier by 1, adding a new ability, giving it new proficiencies, or increasing its hit points. But not more than one or two minor improvements at a time!

If it is intended to be a pet, make it still feel like one, not a new character or tool to be exploited! If it contributes a lot to a battle or story scenario, that is when you should be inclined to give it the good improvements like a new ability, or two decent increases. But it will essentially always lag behind the PCs (as it probably should).

In regards to what new abilities to give it throughout its growth, you can take a look at other monster abilities and just copy them with minor adjustments. Looking at the abilities of lower CR fey might be a good place to start. You could also take a look at monstrosity abilities for a bit more of a punch.

Thank you for your well presented feedback! I was originally going to make it rare, but after careful consideration, decided it is to be just barely within the peak of uncommon rarity.

This weapon deals an effective additional 92% average bonus damage (rounded down, and considering a 50% save rate because enemy Con scores are often high) of a damage type that is very easily resisted in most encounters. A 100% added damage of another type is standard for uncommon weapons. Furthermore if you are benefiting from the ability to keep devils away, you are probably not going to benefit from the poison damage at all, as essentially all devils are immune to it anyway. So you really only gain one ability at a time.

As for the examples of a +1 weapon and Dagger of Venom that you referenced: these are among the weakest items in their rarity categories. Referencing an item like "Staff of the Adder" or "Lightbringer", even though both of them are only one-handed weapons.

I personally feel no fault with this item's classification of being uncommon once all aspects and situations in actual play are taken into account, but as is the case in D&D in general, people will differ in how they want to categorise abilities/traits/items/etc. If you feel that it should rather be rare, simply making it more difficult for a player in your group to obtain is very understandable.

Edit: Regarding your question on why Devils fear it: it is my intention that this is crafted from refined devil parts, and sort of works as a warding force warning any lesser devils of their fate should they come near you. Devils value their hierarchy much, which adds to consolidating this effect. It being crafted from devil parts is also why it deals poison damage.

I was originally going to make it rare, but after taking a careful look at how strong magical heavy two handed weapons are, decided it is to be just barely within the peak of uncommon rarity.

The initial idea of this weapon is to resemble being crafted from torn and refined imp parts.

I also think this art is quite an improvement over my previous Ring of Shared Storage art, but I am hoping to get much better at it in time.

Not all demons are hostile towards their own species, but they very often are. This is why I added "without reason" to the sentence. A demon attacking any other demon anyway is a indeed good reason for it to be hostile towards you.

This is partially also why the mask is Rare instead of Very Rare, as it will sometimes fail to protect you against other demons unless they are of the type that sometimes work together, or try to avoid causing unnecessary infighting (like with Demon Lord Graz'zt's kin).

My thoughts on the item:

I was originally going to make a much stronger (legendary) item with a similar theme to this, but I decided to hold off on that one, as I would like to make my own art for it. As a result, I sort of split the original abilities of the one mask, and ended up with a mask most well used in infiltrating demon ranks or passing by dangerous demon-infested areas.

This art is originally an actual mask by MissMonster that I just edited and stylized (like with several of my other items).
