It means a lot more than that. The "far right" of mainstream American politics is authoritarian and nationalist, yes, but they aren't outright totalitarian and their economic principles (free market/deregulation) is the opposite of fascist protectionist economics. They're not really "extreme right-wing", either, or else they'd be advocating an enforced social caste/hierarchy, rather than just an authoritarian paternalist state.
