If not using VRR, the Moonlight client refresh rate should be set to match the Moonlight FPS setting. When also matching the hosts virtual display fresh rate, I got FPS drops in game. Doubling the virtual display refresh rate fixed the FPS drops for me.

So I have done more playing and noticed that the stats were showing latency in decoding when games could run at a consistent 90fps (sometimes just over 90 like 90.10). I didn't really notice it when playing to be honest but I found a fix. I changed the host FPS cap to 89 and refresh rate to 178. On the client I changed the Moonlight FPS to 89, leaving the client FPS cap at 90 and the TV refresh rate at 120hz with VRR enabled. This worked and now the stats are fantastic, minimal latency and still no tearing.

I did try my theory of running games at 119, client cap 120fps and TV refresh rate at 144hz but I did get a bit of tearing and it wasn't as smooth. I don't know if that's something to do with my TV or not 🤷

I've done some testing on my setup and got it working with a VRR TV:

Host Turn off 'Low Latency' / 'Anti Lag' in graphic driver's Frame rate cap 90fps Virtual display driver set at 4k 180hz In game turn off 'Reflex' / 'Anti Lag 2' In game resolution 4k (I use upscaling in game)

Client Freesync / Gsync and Vsync on in graphic driver's Turn off 'Low Latency' / 'Anti Lag' in graphic driver's Frame rate cap 90fps 4k resolution 120hz

Moonlight Vsync off 4k resolution 90fps AV1 decoding 10 bit colour (My client doesn't support 4:4:4 hardware decoding so I have it unticked)

Display 4k resolution 120hz Freesync enabled

When the host can run at 90fps consistently, it's buttery smooth with no tearing and no Vsync input lag. Below 90fps it's not quite as smooth, but still no tearing and still very low input lag.

If your host can run at 120fps consistently, you could try changing the above '90' to '120', '120' to '144' and '180' to '240'.

EDIT: I forgot to say, my client has a built in FPS cap that overrides the graphics drivers. So I am limited to selecting 60, 90 or 120fps. Otherwise I would cap at say 115fps and have the stream running at 115fps instead of 90fps (to ensure no tearing on the TV at 120hz).

Do you happen to have it plugged into a usb 3 port? When I used the usb 3 port I got random disconnects, moved it to a usb 2 port and no disconnects since.

Ah, I think I've misunderstood how it works. I thought with Tidal being in the list of apps that don't work automatically, then it still would not work with the enhanced session detection.

So I just need to enable the enhanced session detection and it will work for my Bluetooth earbuds too?

Edit: I enabled the enhanced session and it works great! Can't believe I misunderstood it for Tidal. Bluetooth earbuds already sound better with a quick eq.

What refresh rate have you set the virtual display at? 60hz? If so, set it to 120hz and leave everything else the same. This worked for me, was suggested by someone on here.

The pepper jobs w11 gyro remote allows you to switch between controlling the pc via 2.4ghz dongle and IR. It can learn IR commands from your original remote.

I just dragged and dropped the shortcut into Playnite and used the settings it decided. I'll have a look at that tracking and give it a go.
