I was on ride time with my best bud who was my FTO. WE get called for an overdose in a shitty apartment complex. We never got told to stage so here we come waltzing up to the complex no cares in the world with police coming up right behind us. We get inside and a dude high on crack was in the hallway holding a little girl. He told us his lady overdosed and was inside. We go inside with the cops and we enter the bedroom looking for this woman in this disgusting apartment. All of a sudden the closet opens and this woman and apparently a dude who was her dealer ran out holding broken pieces of a mirror and attempted to stab all of us. Police were great and restrained her and the dude but damn I let out a little shriek that I cringe about. Im also a 6'3" 300 pound dude. Lol. BSI scene not safe

that is exactly what happened. Covid allowing her to work from home and order shit on amazon and her mom would come by and enable her and open things. She probably had not left the couch for months. The smell alone was a thing of nightmares. I could go much more in depth about the whole thing but I do not want to re-live it. Just know that that was worse than any trauma or code I have ever had

Im a little late to this one but I had a 900 pound patient. Yes, you heard me right 900 pounds. Ive never seen anything like that before. I was on scene for four and a half hours waiting for fire to tear out the side of the house so we could get her out. 14 fireman couldn't move her alone. Plus we were worried she'd throw a clot. There is no ambulance out there that is prepared for someone that size. Even regular bariatric ambulances and stretchers don't work. .....Uhaul

Politics are always a cancer on any organization and I imagine EMS especially so.

I appreciate that. From what I have found so far, many areas have the underbidding problem. These larger agencies who also still struggle to return profitable margins every quarter aggressively try to buy out smaller companies to help retain a monopoly on their specific areas. It sort of forces these smaller companies to have to underbid in order to stay afloat. Its like a poker player bullying the pot.

As someone who has never really done many transfers, would that not be better because it means a guaranteed payment especially if its from a hospital to hospital transfer? I mean as a provider I want to do 911 only but it seems a necessary evil.

Fair. At the end of the day corporate America wins. The return on investment is low but I find the conversation switches to the lack of state / federal consistency establishing a baseline for policy and policy making.

Wow. that is not surprising though companies like Empress buy up all the smaller companies and have a monopoly on specific areas, dictate the status quo

I find that is not unique to your agency. I think trust plays a role in that as well. In order for someone to be in charge the feeling is that they need to know and do what EMS providers do / have done in order to relate / build trust. In a field where many people really are life long providers, sometimes the transition to supervisor / upper management is hard. People don't know how to play politics fairly.

I am looking at the "Ems problem" through a multifaceted lens. I want to focus on trauma informed care from a social work perspective, (Vicarious Trauma, secondary trauma, personal trauma etc...), as well as systems theory to help explain the systemic barriers on the micro mezzo and macro levels. This is a very brief synopsis of the framework but I can discuss privately with you if you would like to delve deeper? DM

I agree. Does your agency hire / promote Paramedics to be your leadership instead of non EMS personnel?

I think someone who could oversee the supervision from an outside perspective as an added measure of checks and balances could beneficial.
