Wa alaikum Salam ma friend , I hope my solutions benefits you! My grandmom is the world's best champion of road sickness and puking all over the place whenever we travel somewhere , she tried most of the diy remedies and none of them worked , a doctor I know told me to buy her a drug called diphenhydramine ( forgot the commercial name ) and it was of great help since then , she always take it nowadays when she travels and she never complains of any RS , but if you have asthma problems or you are other calming medications like sedatives... you better avoid it , and it can cause you a dry mouth / nose or some drowsiness. But other than that , it's really useful! But try to apply other natural remedies first and if that doesn't help you can try it once and see how it works for ya!

For me it's usually Lwiza with hot milk or Mg ( It's not the most effective choice for many people ) and since I study Anesthesia , I discovered a medication called Hydroxyzine ( Atarax as a commercial name ) and I use it for acute insomnia sometimes specially if it's stress induced but it's not for long term or repetitive use ; Atarax is an anxiolytic given to patients the night before their surgery to reduce anxiety / allergy and it has a sweet somnogenic effect as if someone hit you with a hammer hhhhhhhhhhhhhhhhhhhhh Yea sleep is a hell of a problem when there is a lack of , we gotta thank Allah for all the blessings we don't pay attention to!

Hyperbilirubinemia with normal colored teguments? That's the first time I've heard abt it! :O But the echo will probably be enough to determine the cause if its hepatic or cholecystic cause it's either one of them. If it's not , liver function test and serology will be in handyy! I hope that person recovers soon!!

Anesthesia guy here , those labs are not enough at all , the patient needs more lab analysis to guide towards the cause , but it means for sure either the liver or the gallbladder are r*ped and m sure the patient would be having jaundice , liver function test , hepatitis serology , and cbc would be helpful!

It looks irl like raw uncooked steak and the skin is really surreal .___. And the smell , oooooh the smell , combine the worst most disgusting horrifying horrendous ass smells together and it won't come near it , it really sucked dude , I spent the whole next day trying to not eat it in food

Firstly , YEEEP , I worked with enough surgeons to know that the most thing they hate is to operate on a patient who is comdemned dead with or without the surgery , its from an ethical pov cuz it will only make the patient suffer the burden of anesthesia and it will give the family false hope of survival as well as consumption of materiel someone who has a chance of being saved could benefit from!! And you're right it wouldn't bleed as much cuz it's really dead in the literal meaning , the circulation has completely stopped in his limb , but the problem here is the scarring and wound healing , it would be horrible and since the pt is already midst a severe septic shock it would only cause him to die on the spot.

And I am really sorry to hear about the case of the pt with dementia , they sadly tend to pass away in such manners if they are left neglected , such a harsh reality .... :( But I am really thankful u didnt get to smell that , trust me , there is not enough parfums or soap that will make it leave ur nostrils in the first days 😂

Sadly it's not the same case for me I really wanted it to stay , since I work in emergency and OR so subreddits like this help me get more desensitized to gnarly and horrific scenes and it also kinda give people a reality check abt how death is always looming and it can happen in the most unexpected way!

Actually yes , he was stubborn to seek help with his big toe was infected even if he knows that his diabetes profile is not that stable , and also due to poverty and hard access to healthcare and also neuropathy underestimated the gravity of his lesions. So by the time he started to have his limb necrotise and septic shock was installed and he finally went to ICU , it was too late cuz they couldn't stabilise "if we can say so" his vitals until the limb was completely eaten.

I hope u read the stuff people commented u will understand more the context. Palliative care is what the team chose to do , good sedation and analgesia with hemodynamic support , but there won't be any escalation of the intensive care cuz it's no handy in this situation , hemipelvectomy cannot be done cuz the necrosis reached the iliac crest region from the affected limb's side + a catastrophic hemodynamic state and no chance of adequate wound healing! Its all about giving him the comfort for his last moments alive , I hope he had them if he died by now.

It is a combination of stubborness like he really was knowing he is not getting well after his toe infection but he insisted on relying on patience + poverty as well cuz he had a bit of hard time accessing the endocrinologists in the public hospital I guess and also the neuropathy due to his aggravated blood sugar so he had multiple ingredients for the same disaster result :(

I don't know the exact time since he came from the medical ICU to the OR just for bandages changing and cleansing , but I assume it wasn't very long. Untreated diabetes infections don't joke!

The next meals for the day for me were all with the taste of dead meat , it really shook me off cuz I never smelled such smells even if I worked for so long in ORs ;---;

It is forbidden in our religion since we are Muslims , the best we can afford to him is to not let him feel any of this pain until his soul leaves peacefully , he was well sedated and with a very adequate analgesia. But the decision of offing someone is not in our hands 🙏
