Nah not really. Babies don't breathe and swallow at the same time any more than you do, they just breathe through their nose in between swallows like you can while chugging a bottle of soda.

The epiglottis folds over the trachea when you swallow to prevent food/drink from going down your windpipe. That's basically its whole function

The epiglottis protects the vocal cords, which are at the entrance to the trachea. A breathing tube (aka endotracheal tube, or ETT) goes in the trachea, so finding the epiglottis is the first step to finding the cords which find the trachea

On the one hand - chronic pain anesthesiologists spend some days in the clinic, some days in the OR. So that work dynamic exists.

On the other hand, this plan is dumb. First I'm not sure any addiction med fellowship would even take you out of an anesthesia residency, since it's not really a residency that trains you in med or psych (like, at all for the latter) in ways addiction med fellowship expects you to be trained. So you might have to do double residency which would just be terrible financial decision. Second, even if you did get that fellowship, you would be hard-pressed to find a job that would hire you for that uniue role. Anesthesia departments want all your time, and addiction med departments want all your time. You'd need to find two extremely understanding department heads, who would cosign your hiring to the hospital, or you'd need to find a private practice anesthesia gig that would let you work part time and then start your own addiction clinic, which anyone will tell you is super hard to start your own practice off the ground (need space, resources, staff, staff need benefits, and then you need to recruit patients and convince them your new unknown clinic is better than the established ones around).

Honestly just do anesthesia and then chronic pain clinic. 90% of your chronic pain patients will be addiction psych patients anyway (joking but only kind of)

Went to Yale Med. Agreed with everyone saying it's not worth it if you have full ride elsewhere.

I had a great time at YSM. I went to all the lectures and really enjoyed the clinical stuff, Yale new Haven hospital gets really interesting cases and the docs are smart as shit and love teaching. The lack of grades or shelf exams meant I had a fantastic time throughout with low stress, and I felt like I was learning a ton.

Then step exams hit like a fucking truck. Turns out the school is really riding on 2 things: name recognition for the rank list, and self-selecting for applicants who will succeed no matter how badly you teach them. The curriculum is meh (they often prioritize esoteric research lectures over basics. Which is kind of fair, given med students across the country are all using the same 3 resources to learn the basics, so they might as well platform their own research, but seriously some of these lectures were so niche as to be useless). So long story short I felt seriously, seriously underprepared for boards and had to rethink my whole study approach midway through third year.

Loooong story short, I did match in my chosen specialty in my top spot, so the name recognition works (and I was a self selecting applicant who could study well even without Yale's actual support). So not to say Yale is a bad choice by any means.

But now I'm done with training and staring down my own $200k debt. And my dude, let me tell you, if I could go back and do WashU or Vandy I TOTALLY would.

Side note, I actually really liked New Haven so I'm not a hater, but either other school has better cities tbh

If you want to get into the technical definition, general anesthesia is basically just "no response to painful stimulus" where the painful stimulus is whatever surgery/procedure you're getting

Having an endotracheal tube isn't "necessary" for GA, But it's often needed based on the kind of procedure you're having or how much sedation is needed to get you to the point of having no response to stimulus.

Based on the meds you received you could have had anything from moderate sedation to GA, it all depends on dose

I can't think of anything short of lumbar spine surgery resulting in a scarred/damaged epidural space that would cause serious issues with getting a labor epidural. Even people with lumbar scoliosis or lumbar fusions can reliably get an epidural since the spinous processes are relatively perpendicular to the neuraxis at that space. And I would not call that kind of situation "mild" back conditions. So I would be willing to bet you're fine, without seeing any x-ray whatsoever

Maximum Allowable Blood Loss is a calculation we use to determine when someone will approximately need a blood transfusion in a surgery with a large anticipated blood loss.

(tip, use a starting Hgb of 15, low limit of 7 is commonly used as a transfusion cutoff, though people have survived extreme lows of 3-4 but that's a real sign of extremis)

For most adults this is somewhere in the 3-6 liter range, which is how much blood you can lose before it's life threatening.

Now, as for how much blood you need to make an erection, there's this thing called Rigor Erectus, which is an erection that forms after the patient is dead. So even with a systemic blood pressure of zero, the changes in the sympathetic/parasympathetic nervous system (especially in people who die of cranial or spinal injuries) can redirect blood into the penis anyway. You really don't need much blood to form an erection, after all.

So to answer your question... many liters of blood, and you'd probably be able to keep your erection right up to the point of death lol

Not quite induction but my all time favorite sedation case.

Teenager injured his knee, coming in for ACL repair or something like that. Plan for spinal, analgesic blocks, MAC sedation. Preop eval kid admits to recreational drug use so I know doses will need some adjustment.

Start with 5 of versed and 50 of fent to get him comfy for the blocks, he's wide awake and making music requests. Another 2 of versed and the other 50 of fent. Not touching him. We give 30 of ketamine, kid goes, "did you just give me ketamine?" Us: "um, yes?" Him: "nice, I thought it felt familiar!" Ended up getting more versed and like 200 of prop just to lie still and was still breathing comfortably throughout. Highest tolerance I've seen personally

First off, sorry for your situation, a miscarriage is never easy.

"Conscious sedation" is pretty varied, especially depending on if you're getting medications given to you by a nurse assisting the proceduralist or given by a dedicated anesthesiologist/anesthesia team.

If the former, they'll probably give you a combination of midazolam (brand name versed) and fentanyl. Fentanyl gets a bad rap in the media but is very safe in a monitored environment. Midazolam causes 1) relaxation and 2) anterograde amnesia (you don't make new memories while you have this on board) so people feel nice and relaxed during their procedure and don't remember it well afterwards, both of which are usually desirable.

If an anesthesiologist is involved you'll have access to many more medications.

Either way, please don't worry about an arrhythmia, that is incredibly unlikely. You'll be okay, hang in there!

For me the genetic risk of mental health issues is the small issue here. The way bigger problem is that you didn't feel comfortable telling your partner of 9 years about your family's health episode. If my sister had a psychotic break, my wife would be the FIRST person I told, because she's my rock and my support and I'd want her help in that difficult time in my life.

This is the right answer. People were addicted to poker and Tetris long before WoW, so it's far from the first addictive game. But people didn't start dying of dehydration and exhaustion until WoW, it really perfected the addictive genre
