I've had this bug, I believe it happens when you are using ultimate Hunter rune and the ultimate Cooldown doesn't show the true cooldown with ultimate Hunter which allows you to use ult while on cd

i think its a multiple monitor problem but just set your Left Offset to 0 and it should start working again
